| ObjectiveTo observe the clinical effect of Yangxueqingnao Granule in the treatment of vestibular migraine with blood deficiency and hyperactivity of liver and its effect on CGRP and ET-1,and to propose a new method and idea for the prevention and treatment of this disease.MethodsThe cases came from patients with vestibular migraine who had blood deficiency and liver hyperthyroidism and were diagnosed in the neurology clinic of Chengdu University of Traditional Chinese Medicine Affiliated Hospital(Sichuan Provincial Hospital of Traditional Chinese Medicine)from January 2018 to December 2019.Using random,controlled,single-blind,and non-inferiority scientific research design methods,we divided the subjects into a treatment group and a control group randomly according to a 1:1 ratio,45 cases in each group,and the treatment group was given Yangxueqingnao Granule(oral,1 bag three times a day)and the control group Flunarizine Hydrochloride Capsules(once every night,before bedtime(10mg per night under 65 years old,5mg per night for people over 65 years old)),the total course of treatment is 4 weeks,and the visual analogue score(VAS)and vertigo level are monitored before and after treatment Grading standards,TCM syndrome efficacy scores,number and duration of vertigo attacks,migraine efficacy scores,number and duration of migraine attacks,differences in CGRP and ET-1 content;12-week follow-up visual analog scale(VAS),vertigo severity Grading standards,TCM syndrome efficacy scores,the number and duration of vertigo attacks,and the number and duration of migraine attacks to understand its long-term efficacy.Results1 Curative effectComparison within the group: After treatment,the stun levels,visual analoguescores(VAS),vertigo duration and number of episodes,TCM syndrome scores,migraine episodes,and duration mean were lower than those before treatment.The difference between the two groups was statistically meaning(P <0.05).(1)Two groups are compared after treatment: After treatment(4 weekends),the two groups were evaluated for vertigo severity criteria,visual analog scale(VAS),vertigo duration and number of episodes,TCM syndrome efficacy score,number of migraine attacks There was no significant difference in the mean of duration(P>0.05).(2)Effectiveness of TCM symptom efficacy score: The total effective rate was89% in the treatment group,and the total effective rate was 84% in the control group.After the rank sum test,the difference between the two groups was statistically meaningless(P> 0.05).(3)Subtractive efficiency before and after migraine treatment: The total effective rate was 84% in the treatment group and the total effective rate was 82% in the control group.After the rank sum test,the difference between the two groups was statistically meaningless(P> 0.05).2 Analysis of changes in serum calcitonin gene-related peptide(CGRP)and endothelin(ET-1)levels of neuropeptidesThe changes of serum calcitonin gene-related peptide(CGRP)and endothelin(ET-1)content in the two groups after treatment were lower than those before treatment,and the difference between the two groups was statistically meaning(P<0.05).There was no significant difference in the changes in serum calcitonin gene-related peptide(CGRP)and endothelin(ET-1)content between the two groups after treatment(4 weeks)(P> 0.05).3 securityRespiratory,heart rate,body temperature,blood pressure,blood routine,liver and kidney function indexes,and adverse reactions before and after treatment between the two groups were not significantly different after the rank sum test(P> 0.05).4 Follow-upComparison between the two groups before treatment(0 weeks)and follow-up(12 weekends): grading criteria for vertigo,visual analogue score(VAS),duration of vertigo and number of episodes,TCM syndromes The curative effect score,the number and duration of migraine attacks were all lower than the mean before treatment(0 weeks),and the differences were statistically meaning(P <0.05).Comparison between the two groups after follow-up(12 weekends): after the two groups(12 weekends)follow-up,the criteria for vertigo grading,visual analogue score(VAS),duration of vertigo and number of episodes,scores of TCM syndrome efficacy,There was no significant difference in the number and duration of headache attacks(P> 0.05).Conclusion:(1)The levels of CGRP and et-1 in vestibular migraine increased before the vertigo treatment,and decreased during the intertemporal period after treatment,consistent with the characteristics of migraine,suggesting that vestibular migraine is a subtype of migraine.(2)Yangxueqingnao Granule can improve the symptoms of vestibular migraines with blood deficiency and hyperactivity of liver.(3)Yangxueqingnao Granule can regulate the dynamic balance of CGRP and et-1in blood of patients with vestibular migraine with blood deficiency and liver hyperactivity.(4)The therapeutic effect of Yangxueqingnao Granule on vestibular migraine with blood deficiency and hyperactivity of liver was not worse than that of Flunarizine Hydrochloride Capsule.(5)The safety of Yangxueqingnao Granule is better. |
