Objective: By the clinical observation of Zhi Zhu Sheng Jiang Decoction in the treatment of functional constipation(spleen deficiency and qi stagnation syndrome),explore its safety and effectiveness,so as to provide a new idea for the treatment of this disease. Methods: Collect 72 patients who came from the department of gastroenterology of affiliated hospital of Chengdu University of TCM and met the inclusion criteria of functional constipation of spleen deficiency and qi stagnation syndrome. They were randomly distributed into two groups. The treatment group consisted of 36 patients and the control group included 36 patients.Western drug therapy(polyethylene glycol 4000 powder)was used for the control group while Zhi Zhu Sheng Jiang Decoction was used in the treatment group. The course of treatment was 4 weeks. Before and after treatment for 2 weeks and 4 weeks, record and compare the total score of main symptoms of constipation and the score of single symptom of constipation,TCM syndrome total score and TCM single syndrome score, and finally compare the efficacy of main symptoms of constipation and syndrome of TCM after treatment for 4 weeks in the two groups to evaluate the clinical therapeutic effect and record the adverse reactions. After 2 months, the recurrence of the effective patients was recorded. The datas were analyzed by the medical statistical software SPSS26.0.Results:(1)Assessment of curative effect of main symptoms of constipation: the total effective rate of the treatment group and the control group were 94.29% and 87.50% respectively, showing that the treatment group has an advantage over the control group(P < 0.05).(2)Evaluation of therapeutic effect of Tranditional Chinese Medicine Syndrome: the total effective rate of the treatment group was 91.43%,and that was obviously preferable to the control group(P < 0.01)when comparing with 84.38% of the control group.(3)Main symptom scores of constipation: the two groups can significantly improve the main symptom scores of constipation after treatment for 2 weeks and 4 weeks(P < 0.01),and in reference of improving the main symptoms of constipation, the control group had a lower effect than the treatment group(P < 0.05).The curative effect was better in both groups at 4 weeks than at 2 weeks(P < 0.01).(4)Total scores of TCM syndrome: both groups can significantly improve the total scores of TCM syndrome after treatment for 2 weeks and 4 weeks(P < 0.01),and the treatment group excelled clearly(P < 0.01).The curative effect was better in both groups at 4 weeks than at 2 weeks(P < 0.01).(5)Single symptom score of constipation: after treatment for 2 weeks and 4 weeks, the fecal characteristics, defecation difficulty, defecation time, falling, endless, distension feeling and defecation frequency of the two groups were distinctly boosted(P < 0.01).The treatment group was apt at improving defecation difficulty and fecal character than the control group(P < 0.05)when treated for 2 weeks;in defecation time, falling, endless and distended feeling, defecation frequency, the two groups were equally effective(P > 0.05).when treated for 4 weeks, the treatment group was obviously preferable to the control group with respect to improving defecation difficulty(P < 0.01);in fecal character, defecation time, falling, endless and distended feeling, the control group was not as good as the treatment group(P < 0.05);in defecation frequency, the two groups had not changed obviously(P > 0.05).In both groups, the curative effect of 4 weeks of treatment surspassed that of 2 weeks of treatment(P < 0.01 or P < 0.05).(6)Single syndrome score of traditional Chinese medicine: after treatment for 2 weeks and 4 weeks, defecation weakness, abdominal distention, abdominal pain, weakness after defecation, mental fatigue, anorexia, bowel sounds, sagittal Qi,chest and flank fullness and belching of the two groups were significantly improved(P < 0.01).After treatment for 2 weeks,in the improvement of defecation weakness and post defecation fatigue, the control group was not as good as the treatment group(P < 0.05);As for abdominal distention, mental fatigue, anorexia, sagittal Qi,abdominal pain, bowel soundschest and flank fullness, belching, the treatment group was not clearly differentiated from the control group(P > 0.05).When treated for 4 weeks,in the improvement of defecation weakness and abdominal distention, the control group was significantly inferior to the treatment group(P < 0.01);regarding post defecation fatigue, mental fatigue, anorexia, bowel sounds, and sagittal Qi,the treatment group overweighed the control group(P < 0.05);in abdominal pain, chest and flank fullness, belching, the two groups were equally effective(P > 0.05).In both groups, the treatment effect of 4 weeks was superior to that of 2 weeks(P < 0.01 or P < 0.05).(7)Safety analysis:Before and after the treatment, there were no obvious abnormalities in the patients' general vital signs, blood routine, urine routine, stool routine, liver and kidney function, electrocardiogram. There were light adverse reactions in the treatment and control groups, and the adverse reaction rates 2.86% and 9.38% respectively without remarkable distinction(P > 0.05).(8)Following up: after two months following up,when it came to the recurrence rate, the control group and the treatment group were 43.48% and 16.67% respectively, which suggest that the control group increased the odds of the recurrence than the treatment group(P < 0.05).Conclusion: Zhi Zhu Sheng Jiang Decoction has a good clinical effect in the treatment of functional constipation(spleen deficiency and qi stagnation syndrome),especially in defecation difficulty, defecation weakness and abdominal distention. It was safe and its recent recurrence rate and adverse reaction rate were low.
