Clinical Study On Electroacupuncture And Traditional Chinese Medicine Combined With Pelvic Floor Muscle Training For Stress Urinary Incontinence In Perimenopausal Women

Objective: To study the clinical effect of electroacupuncture and Chinese medicine combined with pelvic floor muscle training on perimenopausal women with stress urinary incontinence.Methods: From October 2016 to February 2018,221 patients with mild to moderate SUI diagnosed by visiting the perimenopausal outpatient department of Xinhua Hospital,Zhejiang Province were selected.In the end,202 patients(control group n=66,treatment group 1 n=68,treatment group 2 n=68)completed treatment and follow-up.The control group was given Kegel exercise training,treatment group 1 received electrical stimulation biofeedback therapy on the basis of the control group,after TCM syndrome differentiation,treatment group 2 was given the addition and subtraction of Buzhong Yiqi Decoction and electroacupuncture on the basis of control group.Patients in the three groups were followed up regularly for 6 months after treatment.At the end of,3 and 6 months of treatment,instruct patients to take 1 h urine pad test,fill out the international incontinence advisory committee on urinary incontinence questionnaire summary table(ICI-Q-SF),at the end of and 6 months of treatment,take urine dynamics test,and evaluate the curative effect.Result: 1)The ICI-Q-SF scores in three groups were significantly lower at the end of treatment and 3 and 6 months after treatment than before treatment(P < 0.05).At the end of treatment,3 and 6 months after treatment,the treatment group was significantly lower than the control group(P < 0.05).There was no significant difference between the two groups at the end of treatment,3 and 6 months after treatment(P > 0.05).2)At the end of treatment,3 and 6 months after treatment,Leakage of urine in 1h urine pad test were significantly lower than before treatment(P < 0.05).Treatment group 1 was significantly lower than control group at 3 and 6 months after treatment(P < 0.05).At the end of treatment,3 and 6 months after treatment,treatment group 2 was significantly lower than control group and treatment group 1(P < 0.05).3)ALPP was significantly higher in the three groups at the end of treatment and 6 months after treatment than before treatment(P < 0.05).At the end of treatment and 6 months after treatment,the levels of the two groups were significantly higher than those of the control group(P < 0.05).There was no significant difference between the two groups at the end of treatment or 6 months after treatment(P > 0.05).4)MUCP was significantly higher in the control group at the end of treatment than before treatment(P=0.003).At the end of treatment and 6 months after treatment,the levels of the two groups were significantly higher than those before treatment and the control group(P < 0.05).There was no significant difference between the two groups at the end of treatment and 6 months after treatment(P > 0.05).5)Qmax was significantly lower in the three groups at the end of treatment and 6 months after treatment than before treatment(P < 0.05).Treatment group 1 was significantly lower than the control group at the end of treatment and 6 months after treatment(P < 0.05).At the end of treatment,treatment group 1 was significantly lower than treatment group 2(P=0.049).There was no significant difference between the two treatment groups 6 months after treatment(P=0.052).Conclusion: Electroacupuncture and traditional Chinese medicine combined with pelvic floor muscle training in the treatment of SUI in mild to moderate perimenopausal women can reduce urine leakage,increase abdominal pressure leakage point pressure(ALPP),maximum urethral closure pressure(MUCP),reduce maximum urine flow rate(Qmax),and significantly improve the quality of life of the patients.
