Objectives With the improvement of clinical diagnosis and treatment level,the survival period of breast cancer patients has been greatly improved,and the quality of life of patients after operation has become an important issue.The purpose of this study is to investigate the difference of postoperative quality of life in patients undergoing BCS,breast reconstruction with expander implantation and MRM.Methods Retrospective analysis of the breast cancer patients from in the Department of glandular surgery of Hebei General Hospital from September 2017 to June 2019.All the patients were treated in accordance with the 2017-2019 NCCN guidelines.The selection of surgical methods was based on the requirements of the guidelines and the wishes of the patients.According to different operation methods,patients with breast cancer are divided into BCS group(n=34),breast reconstruction with expander implantation group(n=9)and MRM group(n=62).To collect the clinical data of breast cancer patients.Including: 1 general clinical data of patients: age of onset,postoperative follow-up time,tumor stage,education level,marital status,annual income,chemotherapy,radiotherapy,targeted therapy,endocrine therapy,local recurrence or distant metastasis or death during followup.2 The quality of life of breast cancer patients is evaluated according to the scores of physiological part,social family status,emotional status,functional status and additional attention included in the Chinese version of FACT-B.3 According to the ZBI,the quality of life of the spouses of the three groups of patients is evaluated.By comparing the general clinical data of the three groups of patients and the scores obtained through the questionnaire,the quality of life of the three groups of patients and their spouses are statistically analyzed.Results A total of 105 patients completed the questionnaire,including BCS group(n=34),breast reconstruction with expander implantation group(n=9),MRM group(n=62).According to statistical analysis,the median age of onset of the three groups is 45 years old.The median postoperative follow-up time is 16 months.The local recurrence rate is 0%,and the survival rate is 100%.There is no significant difference in the age of onset,postoperative follow-up time,tumor stage,education level,annual income and marital status,local recurrence rate and survival rate among the three groups(P>0.05).Five aspects of quality of life in three groups of breast cancer patients were investigated according to the quality of life scale of breast cancer patients FACT-B in Chinese version.The overall score showed that: BCS group(105.56±5.78),breast reconstruction with expander implantation group(104.22±11.96),MRM group(95.40±11.00).According to ZBI scale,69 spouses completed the questionnaire,including BCS group(n=21),breast reconstruction with expander implantation group(n=5)and MRM group(n=43).The overall scores : BCS group(37.67 ± 8.46),breast reconstruction with expander implantation group(38.00±6.60),MRM group(44.19±10.87).After statistical analysis,the results show that there is statistical significance in the overall score of quality of life of the three groups of patients(P<0.05).There is no significant difference between BCS group and breast reconstruction with expander implantation group(P>0.05).There is significant difference between BCS group and MRM group(P<0.05).There is a significant difference between the breast reconstruction with expander implantation group and MRM group(P<0.05).The total score of quality of life of spouses in the three groups is statistically significant(P<0.05).The total score of quality of life of spouses in BCS group and MRM group is statistically significant(P<0.05).There is no significant difference in the overall score of quality of life among the patients' spouses in the breast reconstruction with expander implantation group and BCS group?MRM group(P>0.05).Conclusions 1 The quality of life of breast cancer patients receiving BCS and expander implantation is better than that of patients receiving MRM.2 The quality of life of the spouses in BCS group is better than that in MRM group.3 In order to improve the quality of life of breast cancer patients after operation,we should strengthen the communication between breast cancer patients who are suitable for breast conservation and reconstruction.Figure3;Table7;Reference 169...
