| Objective:The purpose of this study was to retrospectively analyze 482 cases of patients with benign uterine diseases treated by surgery in our hospital.The observation group was 262 cases of total hysterectomy combined with bilateral salpingectomy,and the control group was 220 cases of total hysterectomy.To study the differences of two different surgical methods in patients’ general data,surgical conditions and postoperative recovery,as well as the postoperative perimenopausal symptoms(improved Kupperman score),postoperative pelvic diseases and the occurrence of secondary surgery,etc.,so as to provide theoretical basis for the future surgical treatment of benign uterine diseases.Methods:A retrospective study was conducted on the patients treated in the fourth hospital of hebei medical university from October 1,2005 to October 1,2015,and the patients who met the inclusion and exclusion criteria were selected to have benign uterine disease.Canvassed the patient’s age,body mass index,disease types,size of the uterus,history of previous pelvic surgery,intraoperative bleeding and postoperative hospital stay,pulling the urine tube time,postoperative recovery time of gastrointestinal function,operation related complications,postoperative symptoms of the menopausal transition(modified Kupperman score),the occurrence of postoperative pelvic disease and secondary surgery and other related information,and create the related Excel database.SPSS 25 statistical software was used for statistical analysis of the collected data.The measurement data were expressed as (?) ± s,and the inter-group comparison was performed by t test.Statistical data were compared using x~2 test.The difference of P < 0.05 was statistically significant.Results:1.In this study,there was no statistically significant difference between the observation group and the control group in terms of age,body mass index,disease types,uterine size,and previous pelvic surgery history(P>0.05).2.The two groups were compared in terms of intraoperative bleeding,postoperative hospital stay,urinary catheter removal time,and postoperative gastrointestinal recovery time,and the differences were not statistically significant(P>0.05).There were no operation-related complications in the two groups.3.In all patients,there was no obvious perimenopausal symptoms before the operation.In the follow-up 3 years after the operation,114 patients in the observation group had perimenopausal symptoms,accounting for 43.59%,and 94 patients in the control group had perimenopausal symptoms,accounting for 42.76%.The improved Kupperman score was compared between the two groups 3 years after surgery,and the difference was not statistically significant(P>0.05).4.The differences of postoperative pelvic benign lesions,pelvic malignant lesions and secondary operations between the two groups were statistically significant(P>0.05).Conclusions:Whole uterus bilateral tubal resection at the same time,compared with pure total hysterectomy can reduce postoperative pelvic benign lesions of child-bearing age women(Fallopian tube prolapse、Residual ovarian syndrome、pelvic mass etc),the incidence of pelvic malignant lesions and secondary surgery,does not increase the intraoperative blood loss,postoperative hospital stay,pull out the urine tube time,postoperative recovery time of gastrointestinal function,and had no obvious effect on ovarian function. |
