Objective:To analyze the clinical efficacy of San Fu Tie combined with tiotropium in the prevention and treatment of chronic obstructive pulmonary disease(COPD)with Kidney-yang Deficiency Syndrome.Methods:There were 120 patients of chronic obstructive pulmonary disease in the stable period(traditional chinese medicine syndrome differentiation of Kidney-yang Deficiency Syndrome)that were treated in the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from January 2018 to June 2018 and have long lived in the community near the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine meet the inclusion criteria were taken as the research object.Based on the patient's visit time,they were divided into two groups randomly,the control group and the treatment group.The control group(60 cases)was treated with inhalation of tiotropium bromide powder.The treatment group(60 cases)was treated with inhalation of tiotropium bromide powder,followed by San Fu Tie.Follow-up for one year after treatment.Compare the changes in the St.George Respiratory Questionnaire(SGRQ)score,dyspnea index(m MRC),and the COPD self-assessment test(CAT)questionnaire before and after treatment in the two groups.Analyze the differences in treatment effect,the number of cases re-hospitalized due to exacerbations within one year and the improvement of TCM symptom score between the two groups.Results:The total effective rate of clinical efficacy in the treatment group was 91.67% and that in the control group was 75.00%.The difference in total effective rate between the two groups was statistically significant(P<0.05),and the treatment group was better than the control group.Comparison of the probability of rehospitalization due to the exacerbation of the disease in the two groups within 1 year after treatment:there were 20 cases in the treatment group(41.7%);32 cases in the control group(64.0%),the difference was statistically significant(P<0.05).The patients in the treatment group were lower than those in the control group.No patients died within 1 year after discharge from both groups.After treatment,the TCM symptom scores in the treatment group were significantly lower than those in the control group;FEV1,FEV1 /FVC in the treatment group were significantly higher than those in the control group(P <0.05);Comparison of SGRQ scores between the two groups: The scores of pulmonary symptoms,mobility and disease effects in the treatment group were significantly lower than those in the control group(P <0.05);The ratings and the total scores of CAT in the treatment group were significantly different from those in the control group(P<0.05),the ratings and the total scores of CAT in the treatment group were lower than those in the control group after treatment;The 6MWT in both groups was significantly increased compared with that before treatment,and the increase in the treatment group was higher than that in the control group,the difference between the two groups was statistically significant(P <0.05).Safety monitoring: The overall incidence of adverse reactions in patients during the treatment was 20.8%,including allergies to the patch,epidermal burns and broken infections,allergic dermatitis,dry mouth,constipation,palpitations,and difficulty in urination.Adverse reactions were relieved after symptomatic treatment,and no drug-related deaths occurred.Conclusions:The application of San Fu Tie to patients with chronic obstructive pulmonary disease with kidney-yang deficiency syndrome can not only improve the total effective rate of treatment,but also has a good effect in reducing the number of acute exacerbations of COPD in patients in one year,achieving good results in improving its lung function and clinical symptoms.In the treatment of COPD stable phase with San Fu Tie,no obvious intolerable adverse reactions occurred.
