A Study On The Relationship Between TCM Syndrome Types,TCM Constitution And Immune Mechanism,and Some Cytokines Of H Type Hypertension

Objectives:1?To test serum Ig A?Ig G?Ig M?Ig E? complement C3? complement C4?TNF-??IL-6?ET?NO,and analysis of factors related to serum Hcy,and use Logistic regression analysis to analysis whether the Hcy was related to the increase of cytokines;2? To compare of cytokine differences between H and non H hypertension groups and investigate the relationship between immune inflammatory response and H type hypertension;3?To study the TCM Syndrome in Patients with H type Hypertension;4?To study the distribution of TCM constitution in patients with H type hypertension;5?To compare the differences between serum Hcy and cytokines in patients with different TCM syndrome in H type hypertension;6?To compare the differences between serum Hcy and cytokines in patients with different TCM constitution in H type hypertension.Methods:Total of 316 patients with essential hypertension were included,serum cytokines were measured,and related factors related to serum Hcy were studied.The 316 patients with essential hypertension were divided into 206 patients with H type hypertension group and 110 patients with non H type hypertension group,To study the distribution of TCM syndrome type and TCM constitution in H type hypertension group,and to compare the serum Hcy and cytokines between TCM syndromes and TCM constitution.Results:(1)Serum Hcy was positively correlated with age and TNF-??ET(P<0.05)and negatively correlated with NO(P<0.05).Two Logistic regression analyses showed that Hcy was independently associated with TNF-??ET?NO and was a risk factor for elevated TNF-??ET in patients with essential hypertension and a protective factor for elevated NO(P<0.05).Age is a risk factor for high serum HCy in patients with essential hypertension(P<0.05).There was no correlation between residual cytokines and serum Hcy in patients with essential hypertension(P<0.05);(2)The Ig G?TNF-??ET of H type hypertension group was higher than that of non-type hypertension group(P<0.05),the NO was lower than that of non-type hypertension group(P<0.05),and there was no significant difference between the remaining cytokine groups(PP<0.05);(3)The TCM syndrome of H type hypertension patients is liver yang hyperactivity syndrome,which is liver yang hyperactivity syndrome > yin deficiency yang hyperactivity syndrome > liver kidney yin deficiency syndrome and yin and yang deficiency syndrome;(4)The main types of TCM physique in H type hypertension group are yin deficiency > phlegm dampness > dampness and heat > yang deficiency,qi stagnation > flat and qi deficiency,intrinsic and blood stasis;(5)Comparison with total mean,Serum Ig M increased in patients with H hypertension(P<0.05);(6)There was no significant difference in serum Hcy between different TCM syndromes in patients with H type hypertension(P>0.05),the complement C3 of liver yang hyperactivity syndrome was greater than that of yin deficiency yang hyperactivity syndrome(P<0.05),and there was no significant difference between the remaining cytokine groups(P>0.05).(7)There was no significant difference in serum Hcy between different TCM physique in patients with H type hypertension(P>0.05),and the complement C3?TNF-? of damp-heat quality was greater than that of phlegm-wet quality and yin-deficiency quality(P<0.05).There was no significant difference between the remaining cytokine groups(P>0.05).Conclusions:(1)The serum Hcy of primary patients was positively correlated with age and TNF-??ET,and negatively correlated with NO.Hcy is a risk factor for increased serum TNF-??ET in patients with essential hypertension,a protective factor for increased NO,and age is a risk factor for increased Hcy in patients with essential hypertension,residual cytokine index was not correlated with Hcy level.(2)The immune inflammatory injury caused by high Hcy and vascular endothelial dysfunction may be involved in the occurrence of H type hypertension.(3)The increase of Ig M in patients with H hypertension compared with the total mean;(4)H type of hypertension TCM syndrome type is mainly liver yang hyperactivity syndrome,liver yang hyperactivity syndrome > yin deficiency yang hyperactivity syndrome > liver kidney yin deficiency syndrome,yin and yang deficiency syndrome.(5)H type hypertension TCM constitution classification is mainly yin deficiency and phlegm dampness > yin deficiency > phlegm dampness > dampness and heat,yang deficiency,qi stagnation,peace,qi deficiency and blood stasis.(6)The activation of complement system is involved in the course of H type hypertensive patients with liver-yang hyperactivity syndrome,but it may not be related to high Hcy and its pathogenic mechanism.(7)The activation of immune inflammation in the body of damp-heat substance in patients with H type hypertension may have no correlation with HHcy,and it is more related to the pathogenic characteristics of damp-heat substance itself.
