Objective:Using metabonomics technology to screen the serum metabolic markers of insomnia with liver depression and spleen deficiency syndrome,provide metabonomics basis for the differentiation of insomnia syndrome with liver depression and spleen deficiency syndrome.Methods:1.Set up the syndrome of liver depression and spleen deficiency(group I),liver depression and fire syndrome group(group J),heart and spleen deficiency syndrome group(group K),and healthy group(group A).Patients were screened according to the diagnostic criteria of insomnia and TCM-related syndromes,10 cases in each group,and10 healthy subjects were screened according to the baseline conditions of the patients.2.Record basic information of 40 subjects,such as gender,age,course of disease,PSQI score,and the main and concurrent symptoms of TCM in the three disease groups.3.Serum samples of 40 patients were collected,and all serum samples were tested by metabolomics full spectrum.The sample data was preprocessed,and the statistical method used univariate analysis and multivariate analysis,and the potential metabolic markers were screened according to the P value,VIP value,and multiple of differences compared between groups.4.Further screening the metabolic markers of liver-stagnation and spleen-deficiency syndrome among the potential metabolic markers initially screened.Through the comparison of I:A,I:J,and I:K among the groups,it was screened out that all of the three comparison groups have Different ions are identified as the metabolic markers of insomnia with liver depression and spleen deficiency syndrome.5.The final screening of the different substances is determined by searching the "Human Metabolome Database"(The Human Metabolome Database,HMDB)to determine its biological information.Determine the metabolic pathways of metabolic markers in the "Kyoto Encyclopedia of Genes and Genomes"(Kyoto Encyclopedia of Genes and Genomes,KEGG).Results:1.Comparing the gender composition of the subjects,there were 3 males in the liver-stagnation and spleen-deficiency syndrome group,liver-stagnation transforming fire syndrome group,and healthy group,and 7 females.In the heart and spleen deficiency group,there was 1 male and 9 females.Chi-square test X2=1.773,P=0.703,comparison between groups is meaningless(P>0.05);comparison of age distribution,liver depression and spleen deficiency syndrome group 36.60±12.12 years old,liver depression and fire syndrome group 38.10±11.97 years old,heart and spleen two The deficiency syndrome group is 41.30±16.80 years those of the heart and spleen deficiency syndrome group,and the comparison between the groups was meaningless(P<0.05).2.The main sleep symptoms of 30 patients in the insomnia disease group selected in this study are difficult to fall asleep,easy to wake up,and difficult to fall asleep after waking up.There are 47 concurrent symptoms.Patients in the disease group with liver depression and spleen deficiency syndrome showed: 10 times of forgetfulness,8 times of upset,8 times of mental fatigue,8 times of fatigue,8 times of fatigue,7 times of bleak complexion,6 times of dizziness,6 times of swelling,5 times of irritability and 5times of dry mouth are the first 10 symptoms of high frequency;patients with liver depression and fire syndrome showed: 9 times of forgetfulness,8 times of irritability,7times of fatigue,6 times of easy startling,dry mouth 6 times,dizziness 6 times,pale complexion 6 times,palpitation 5 times,upset 5 times,heart palpitations 5 times,which are the first 11 symptoms of high frequency;patients with heart and spleen deficiency syndrome show: 9 times of mental fatigue,8 times of forgetfulness,7 times of tiredness,6 times of loose stools,6 times of bleak complexion,6 times of irritation,4 times of mental depression,4 times of fatigue,4 times of appetite,3 times of ecstasy,which are the top 10 frequent occurrences The symptoms are.3.Comparing the liver-stagnation and spleen-deficiency group with the healthy group,a total of 418 potential metabolic markers were screened,including 185 cations and 233 anions;a total of 4770 potential metabolic markers were screened out between the liver-stagnation and fire syndrome group and the healthy group Metabolic markers,including 3739 cations and 1031 anions;comparison between the heart and spleen deficiency group and the healthy group,a total of 1529 potential metabolic markers were screened,including 943 cations and 586 anions.4.On the basis of the above-mentioned potential metabolic markers that have been screened out(a total of 6,717 potential metabolic markers,including 4867 cations and1850 anions),further screen the metabolic markers of liver depression and spleen deficiency syndrome.The three comparison groups(I:A,I:J,I:K)were screened twice for different ions,and 13 metabolic markers were found.The VIP values ??of these13 metabolic markers are all greater than 1,indicating that the liver-stagnation and spleen-deficiency syndrome group has a better partitioning degree between the groups;the P values ??of the comparison between the groups are all less than 0.05,indicating that the differences between the groups are statistically significant;The multiples of difference between the groups are all greater than 1.2,suggesting that the relative concentration of ions in the liver-stagnation and spleen-deficiency syndrome group is increasing.5.13 substances closely related to the syndrome of liver depression and spleen deficiency in insomnia,by querying "The Human Metabolome Database"(HMDB),the substance classification information includes 5 categories,of which 6 substances belong to phenylpropionic acid and Polyketone,three substances belong to lipids and lipid molecules,two substances belong to anisole,and the two substances are lignans and benzopyrans.The 6 phenylpropionic acid and polyketones are:(S,E)-Zearalenone Myrigalon B Myrigalone E liver depression and spleen deficiency syndrome may have "intestinal hepatic circulation and metabolism" disorders.Phenylpropionic acid and polyketones,lipids and lipids,and anisole may distinguish between liver depression and spleen deficiency syndrome and liver depression in insomnia The typical metabolic markers of Huahuo syndrome and heart and spleen deficiency syndrome,and the detection of the above substances in the serum of patients with insomnia can provide metabonomics basis for the differentiation of the syndrome of liver depression and spleen deficiency syndrome in insomnia.
