ObjectiveAs the pressure on modern young people increases, insomnia becomes more common and younger.Western medicine treatment of insomnia presents side effects and serious adverse reactions. For example,long-term use is prone to serious dependence and addiction. However,Chinese medicine treatment is not limited to traditional Chinese medicine treatment, but through the literature can be found that the etiology and pathogenesis is more complicated, the effect is unstable, and easy to relapse. In this study, the clinical efficacy of treating insomnia caused by liver stagnation and spleen deficiency was observed by sputum acupuncture therapy.MethodsThe first part collects the insomnia patients from Prof.Li Jingbo from the Department of Internal Medicine of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine from January 2018 to December 2018,summarizing the factors of syndrome differentiation, syndrome distribution and syndromes and symptoms.Relationship.In the second part,120 patients with insomnia caused by liver stagnation and spleen deficiency were randomly divided into two groups:the treatment group and the control group, with 60 cases in each group. In the treatment group, the subcutaneous cortex,Hegu,Taichong,Zusanli,Zhaohai,Shenmai(both bilateral)and Zhongyu points were selected. The bilateral acupoint groups alternated,14 days for a course of treatment, and 2 days of rest during the course of treatment.A total of 2 courses were performed. The control group, also known as the sham treatment group, was used to treat the skin tone tape(without the needle, but the size of the application was the same as that of the treatment group). The procedure was the same as the treatment group. Both groups were psychologically counseled to relieve the patient' s upset, anxiety, depression and other negative emotions through communication and other forms. Finally, the two groups were evaluated by Pittsburgh sleep quality index(PSQI)and polysomnography.ResultsThe first part of the study found that the distribution of syndromes of insomnia was the highest in the liver and spleen deficiency type,307 cases(20%);followed by 282 cases(18%)of qi and blood deficiency type;218 cases of qi and yin deficiency type(14 cases)%);212 cases(13%)with turbidity block;210 cases(13%)with liver stagnation and blood stasis;204 cases(13%)with liver qi stagnation type;115 cases(7%)with yin deficiency and damp heat type; The syndrome type was 26 cases(2%),including Shaoyang syndrome,Shaoyin syndrome,Yin deficiency Yangshuo type, spleen and kidney yin deficiency type. There is also a relationship between TCM syndrome type and age.Patients with an onset age of less than 30 years old are mainly liver stagnation and spleen deficiency type, while those with an onset age of over 40 years old are mainly caused by liver qi stagnation and qi and yin deficiency.307 patients with insomnia caused by liver stagnation and spleen deficiency, the frequency of symptoms appeared as difficulty in falling asleep(301),easy to wake up(291),pain in the stomach and ankle(203),chest and flank pain(195),diarrhea(191),hernia(184),upset(183),less(176),dizziness(154)and so on.There is also a statistically significant difference in the type and duration of TCM syndromes. The course of disease is less than 5 years, the most common type of liver stagnation and spleen deficiency type, followed by qi and blood deficiency type;the disease course is more than 5 years, the most common type of qi and yin deficiency type, followed by turbidity block type. The second part of the study found that the clinical efficacy of the treatment group and the control group were significantly different after treatment. The total effective rate was 95%in the sputum needle group and 75%in the control group. After statistical treatment,The two groups were statistically significant(P<0.05),indicating that the clinical effect of the acupuncture group in treating insomnia was better than that of the control group.Comparison of various indicators of sleep progress in the two groups showed that the sleep latency, total sleep time, and sleep efficiency of the acupuncture group were statistically significant compared with the control group.(P<0.05)The sleep structure of the two groups was statistically significant(P<0.05)in the N1 and REM phases of the sputum acupuncture group and the control group. The prolongation of the REM phase was beneficial to the improvement of sleep quality. The PSQI total score and the scores of various factors were found to be statistically significant(P<0.05).ConclusionInsomnia patients with liver stagnation and spleen deficiency are more common in young people, and the course of disease is shorter(less than 5 years). In addition to the symptoms of insomnia such as difficulty falling asleep and waking up,the frequency of occurrence is pain in the stomach, chest pain, diarrhea, and belching, and so on.Compared with the control group, the PSG test showed that there was a difference in sleep structure and sleep progress. The PSG test objectively evaluated the diagnosis and efficacy of insomnia. The treatment of sputum acupuncture regulates the yin and yang of the human body through acupuncture and collateral veins, thereby further reconciling the qi and blood and finally achieving the effect of reconciling the camp, improving the therapeutic effect of treating insomnia, and forming a"colon-pulmonary-gas-bloodmpport-insomnia" axis To better solve the clinical symptoms caused by insomnia. And the needle is convenient for carrying, more convenient and safe, and it is a more convenient and acceptable treatment method for the disease.
