Objective: To explore the influencing factors of anxiety and depression in adult epilepsy patients,in order to provide early warning function for epilepsy patients with anxiety and depression,and provide evidence for early intervention.Methods: Patients diagnosed as epilepsy in Neurology Department of the Affiliated Hospital of Qingdao University were collected from November 2018 to September 2019.General demographic data and disease-related information of patients were got from medical records,for example,course of disease,frequency of attack,degree of control,time of attack,etc..Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)were used to evaluate anxiety and depression in patients.According to the diagnostic criteria of the third edition of Chinese standard for classification and diagnosis of mental disorders and the scores of depression and anxiety scale,the patients were divided into four groups: the simple anxiety group,the simple depression group,the group with anxiety and depression at the same time,the group without anxiety and depression.The HAMD score ? 8 and the HAMA score < 14 in depression group;the HAMD score < 8 and the HAMA ? 14 in anxiety group;the HAMD score ? 8 and the HAMA ? 14 in depression with anxiety group;the HAMD score < 8 and the HAMA score < 14 in non anxiety and depression group.Using rank sum test and chi square test to screen the factors that may cause depression and anxiety in epileptic patients.The factors with P ? 0.05 in single factor analysis were included in logistic regression analysis,P ? 0.05 was considered statistically significant.Results: A total of 315 epileptic patients were collected in the study.The patients in the simple anxiety group,the simple depression group,the group with anxiety and depression at the same time,and the group without anxiety and depression were 14,24,79 and 198 respectively.The univariate analysis showed that:(1)there was no significant difference in age,occupation,age of onset,income,education background,family history,history of febrile convulsion,type of attack,manifestation,presence or absence of head MRI abnormality among all groups(P > 0.05).(2)The Gender,residence(urban,rural),seizure time(day,night),degree of seizure control,seizure frequency,presence or absence of generalized tonic clonic seizure(GTCS),types of oral drugs and course of disease may be related,the differences were statistically significant(P < 0.05).The results of the logistic regression analysis showed that daytime seizure(or = 2.954,95% CI: 1.324-6.592),seizure frequency > 1 time / week(or = 19.701,95% CI: 7.711-50.334),GTCS(or = 16.422,95% CI:3.192-84.480),first diagnosis(or = 3.603,95% CI: 1.177-11.034)and drug resistance(or =7.998,95% CI: 2.683-23.845)were risk factors of anxiety and depression in epileptics Element.At the same time,seizure frequency > 1 time / week was the risk factors of epilepsy associated anxiety(or = 9.838,95% CI: 2.182-44.357),as well as the factor of the depression(or = 6.082,95% CI: 1.785-20.721).Compared with women,men were the protective factors of epilepsy patients with anxiety and depression(or = 0.404,95% CI:0.196-0.829).Sleep seizure alone was a protective factor for depression in epileptics(or =0.124,95% CI: 0.015-0.998).Conclusion:(1)Drug-resistant epilepsy,daytime seizure,GTCS,seizure frequency more than once a week were the risk factors of anxiety and depression in epileptic patients;(2)Men with epilepsy are less likely to have anxiety and depression than women;(3)New diagnosis epilepsy is a predisposing factor of anxiety and depression for epileptic patients;(4)The risk of depression was low in epileptic patients with nocturnal seizures only.
