Objective: The causes of implant failure and related influencing factors were analyzed to provide guidance for clinical work.Methods: All implant failure cases at the implant center of stomatological hospital affiliated to China medical university from January 2005 to December 2017 were selected.The causes of implant failure were analyzed,and the influences of the patients themselves,implants,surgical techniques and prognostic factors of superstructure repair on implant failure were evaluated by single factor and multi-factor analysis.Results: From January 2005 to December 2017,a total of 7250 patients received implant repair treatment in our department,and a total of 11 534 implants were implanted.By the end of March 2018,84 patients with implant failure had been recorded,with a total of116 implants lost,and the total failure rate was 1.01%.The application of life table method to analyze the 13-year cumulative implant survival rate is 96.82%,implant failure is mainly concentrated in the postoperative 1 year,the failure rate in 2~5 years decreases year by year,and after 5 years presents an irregular rising trend.The univariate analysis of factors related to implant failure showed statistically significant variables were gender,age,implant site,cause of tooth loss,bone density,implant length,osteotome technique,internal sinus floor elevation and lateral approach of sinus floor elevation(P < 0.05).However,implant diameter and guided bone regeneration were not statistically significant(P>0.05).The significant variables in the univariate analysis were included in the Logistic multivariate regression analysis,and it was found that the other factors were statistically significant except age,gender and osteotome technique.Conclusions: Early implant failure was mainly caused by poor osseointegration,while late implant failure was mainly caused by inflammation or excessive load.The factors related to implant failure were found to be the factors of the patients themselves and surgical techniques.
