Objective : The purpose of this trial was to investigate whether vortioxetine improves the cognitive function of patients with first-onset depression as a multimodal action drug,and whether it has an advantage over other single-action drugs(such as fluoxetine).Methods:Sixty patients with clinically new depressive disorder were selected as the research object,and they were divided into Vortioxetine group and fluoxetine group according to the random number table,and put 30 people in each group.The severity of clinical symptoms was evaluated using the Hamilton Depression Scale(HAMD-17).Using the Wechsler Memory Scale(WMS)and the Wisconsin card sorting test(WCST)to evaluate the subject's long-term memory,short-term memory,transient memory,executive function and other cognitive functions.After treating patients with depressive disorders for 8 weeks(assessing them in every 2 weeks),comparing their own changes in the degree of depressive disorders before and after treatment,and explore the clinical efficacy of improving cognitive function in patients with first-episode depressive disorders before and after treatment.Results:1.In terms of changes in HAMD scores of the two drugs,the HAMD scores of the two groups improved in every 2 weeks,and the difference was statistically significant(p <0.001);at 0 and 4 weeks,the Vortioxetine group.The difference in the HAMD score between Vortioxetine group and the fluoxetine group was not statistically significant(p> 0.05);at 8 weeks,the decrease in the HAMD score of the Vortioxetine group was lower than that of the fluoxetine group,the difference is statistically significant((F =(2,58)11.285,p <0.001).2.After 8 weeks of standard treatment,comparing the reduction rate of HAMD score between the Vortioxetine group and the fluoxetine group,> 90% is in the recovery group,60%-89% belongs to the significant improvement group,30%-59% were the improvement group,and <30% were the ineffective group.After comparison,the Vortioxetine group had a higher rate of conversion(the recovery group + the significant improvement group + the improvement group),p <0.05,the difference is statistically significant.3.In the Wisconsin card classification test,compare the correct number(CR),error number(TE),andnon-persistent error number(NPE)at week 8 between the two groups.It can be found that after 8 weeks proper appropriate treatment,the improvement of the correct number(CR)of the Vortioxetine group compared to the fluoxetine group is meaningful,that is,the Vortioxetine group has an advantage over the control group in terms of improving the correct number(CR);the error number(TE),and non-persistent errors(NPE)were also has a difference in the improvement,but the difference was not statistically significant(p = 0.507,p = 0.759);4.In the Wechsler memory scale test,the long-term memory(100-1)and short-term memory(picture recognition)of the two groups at week 8were compared,the Vortioxetine group has a significant improvement over the fluoxetine group,p <0.05,the difference is statistically significant;transient memory also has a difference between the two groups after treatment,but it is not statistically significant(p = 0.337).5.In terms of adverse reactions,the adverse reaction rates of the two groups were comparable.Conclusions : 1.Vortioxetine showed better improvement in some cognitive dimensions,and its clinical effect was more obvious than that of fluoxetine group;2.In terms of adverse reactions,the adverse reaction rates of the two groups are equal.
