Background:Frailty(F)in the elderly refers to the decline to physiological reserve capacity and anti-stress ability.And frailty syndrome(FS)refers to the clinical syndrome that is unique to the elderly,which performs to decline of multiple physiological systems and leads to the decline of physiological reserve ability and the decline of anti-stress ability.Vitamin D(Vitamin D: VD)in serum is a steroid derivative,which plays an important roles in living activity of calcium and phosphorus metabolism,neuroendocrine system,immune inflammation and musculoskeletal system.However,with the study of geriatric diseases and frailty,many studies suggest that the low vitamin D level in serum is closely related frailty syndrome,and the serum 25-hydroxyvitamin D level in healthy population is higher than frailty population.In addition,increasing vitamin D can improve the body's muscle function,physical activity ability,and then affect the frailty phenotype.And many studies prove that frailty in elderly is related to vitamin D.But the research in China about FS and VD is also in the infancy and very seldom.Therefore,the research mainly analyzes relationship between frailty status and vitamin D in elderly inpatients in Guangzhou,deepens the clinician's understanding of them and provides clinical evidence.Objective:To study frailty status and the distribution of vitamin D,and the relationship between them in elderly inpatients in Guangzhou Materials and methods:This study collected 129 elderly inpatients aged ?60 years and recorded clinical data(including height,weight,body mass index(BMI),chronic disease,smoking history,grip,pace,weakness,pace loss,weakness,physical activity loss,weight loss,exhaustion,vitamin D level and etc.).T Test and non-parametric Test were employed to frailty status and the distribution of vitamin D in the elderly.And Spearman rank correlation and Logistic regression was used to analyze relationship between vitamin D level and frailty in the elderly.Results:In the study,there were 27 cases(20.9%)in non-frailty,60 cases(46.5%)in pre-frailty and 42 cases(32.6%)in frailty.There were statistically significant difference in pace loss,weakness,physical activity loss,weight loss,and exhaustion in different frailty states(Kruskal-Wallis H(K)=76.882,64.925,63.89,17.417,29.768;P<0.05).There were statistical differences in hypertension,diabetes,stroke and osteoporosis in different frailty states(Kruskal-Wallis H(K)=29.768,6.386,9.256,6.744,P=0.005,0.041,0.01,0.034).In the study,the average value of vitamin D in elderly inpatients was(27.56 10.65)ng/mL.There were 56 cases(43.4%)with sufficient vitamin D in serum,35 cases(27.1%)with insufficient vitamin D,34 cases(26.4%)with deficient vitamin D and 4 cases(3.1%)with serious deficiency of vitamin D.There was no significant difference in vitamin D levels between genders(t = 1.587,P =0.115).There were statistically significant difference in age-grouping,weight loss,exhaustion,weakness,pace loss,physical activity loss in different frailty states(t = 10.724,P = 0.000;t = 2.954,P = 0.004;t = 6.555,P <0.001;t = 8.324,P <0.001;t = 7.804,P <0.001;t = 6.012,P <0.001).In the study,there was a negative correlation between frailty and the levels of vitamin D in elderly patients(r =-0.768,P < 0.001).Vitamin D were significant risk factors associated with frailty.Frailty in the elderly was associated with age,weight,BMI,chronic disease,weight loss,exhaustion,weakness,pace loss,physical activity loss,grip,and pace(r = 0.694,P < 0.001;r =-0.256,P = 0.003;r =-0.234,P = 0.008;r = 0.375,P < 0.001;r = 0.344,P < 0.001;r = 0.446,P <0.001;r = 0.674,P <0.001;r = 0.683,P <0.001;r = 0.694,P <0.001;r =-0.520,P <0.001;r = 0.642,P <0.001).Conclusions:1.The prevalence of frailty syndrome is 32.6% in elderly inpatients in Guangzhou.The incidence of frailty is commonly seen in various chronic diseases.The distribution of FP in different frailty status is different,and the chronic diseases are also different.Therefore,early screening frailty is very important for chronic disease management.2.The level of vitamin D in elderly inpatients in Guangzhou is 27.56ng/ml.The proportion of vitamin D deficiency was 56.6%.Early monitoring of vitamin D level in the elderly is important.3.Frailty in the elderly was negative correlation to the levels of vitamin D.Vitamin D is the independent factor of the frailty.The level of vitamin D was lower,the higher risk of frailty.
