| OBJECTIVESThe objectives of this study are to investigate the present situation of cognitive frailty in hospitalized elderly patients and further analyze the influencing factors.According to the analysis result,We attempt to arouse the attention of the medical staff to the assessment of cognitive frailty in elderly inpatients,provide evidence-based basis for individualized nursing intervention,prevent or delay development of cognitive frailty,improve the quality of life in elderly inpatients,increase the healthy life expectancy and achieve healthy aging.METHODSFrom December 2018 to September 2019,257 elderly inpatients were selected from the department of gerontology ward of a third-class hospital in Zhengzhou.by cross-sectional investigation.The investigation tools included a general information questionnaire,FP(Frailty Phenotype),Mo CA(Montreal Cognitive Assessment Scale),MNA-SF(Short-Form Mini Nutritional Assessment),IADLs(Instrumental Activities of Daily living scale).Epidata3.1 software was used to establish the database and Spss25.0 statistical software was used to conduct descriptive analysis,single factor analysis and regression analysis of the data.RESULTS1.A total of 257 questionnaires were distributed in this research.There were 245 valid questionnaires indicating the effective recovery rate was 92.33%.2.The incidence of cognitive frailty in elderly patients was 43.7%.24.9% of the inpatients were potential reversible cognitive frailty.18.8% of the inpatients were reversible cognitive frailty.3.The average score of instrumental activities of daily living in elderly patients was(12.38±5.67)points.among which reduced ability of instrumental daily living was 62.0%.In descending order,the scores for instrumental daily living ability were housework,outdoor activities,washing clothes,shopping,food cooking,handling finances,taking medication,and using the telephone.4.The average score of nutritional status in elderly patients was(10.99±2.52)points.and 14.3% of them were malnourished,35.1% were at risk of malnutrition,and 50.6% had normal nutrition.5.Univariate analysis show that the influence factors of elderly hospitalized patients with cognitive frailty are age,BMI,cultural degree,marital status,living conditions,hobbies,smoking,medication categories,sleeping,sickly coexist,hypertension,diabetes,coronary heart disease,cardiovascular disease,and other musculoskeletal disease,respiratory disease,instrumental daily life ability,nutritional status.6.The results of ordinal logistic regression analysis showed that age,BMI,educational level,diabetes,co-existence of multiple diseases,instrumental daily living ability and nutritional status are the main influencing factors of cognitive frailty.Among them,age over 75 years old,diabetes,co-existence of multiple diseases,reduced ability of instrumental daily living,malnutrition and risk of malnutrition are risk factors for cognitive frailty;BMI >18.5,education level in junior high school and above are protective factors for cognitive frailty.CONCLUTIONS1.The prevalence of cognitive frailty is at a high level in elderly hospitalized patients and the incidence of potential reversible cognitive frailty is higher than reversible cognitive frailty.Therefore,it is necessary to conduct early identification and intervention.2.Age,BMI,educational level,diabetes,co-existence of multiple diseases,instrumental daily living ability and nutritional status are the main influencing factors of cognitive frailty in elderly hospitalized patients.3.Age over 75 years old,diabetes,co-existence of multiple diseases,reduced ability of instrumental daily living,malnutrition are risk factors for cognitive frailty;BMI >18.5,education level in junior high school and above are protective factors for cognitive frailty. |
