ObjectiveTo explore the influence of nursing assistance on nurses' dimission intention in a county-level hospital under the model of medical consortium(hereinafter referred to as medical consortium).MethodsA total of 325 nurses from a county-level people's hospital of a third-grade hospital group in hunan province who met the inclusion and exclusion criteria were selected as research objects.The researc hers sent questionnaires to the subjects in the form of questionnaire star in January 2019.Each subject was required to complete the questionnaire independently and submit it within a specified time.The res earchers collected questionnaires and analyzed the general demographic data,job satisfaction,occupational commitment and turnover intention of the subjects.In February 2019 in couplet of medical mode,the main unit(the third-grade hospital)for the county-level hospitals car e support,support include:career planning guidance,unified nursing work standards and optimize workflow,implementing flexible scheduling,set up a performance evaluation mechanism,to participate in the man agement of nursing quality control,professional skill guidance.The forms of support include “up and down” and “down” and “one-to-one”psychological counseling.Eight months after the intervention,the qu estionnaire was sent out again in the form of questionnaire star in October 2019 to compare the changes of nurse' job satisfaction,occupational commitment and turnover intention before andafter the inter vention.SPSS22.0 was used for statistical analysis of the data.USES the frequency and percentage to describe general demographic data,USES the mean ± standard deviation to describe nurses job satisfaction,career commitment,leaving intend each dimension score,different demographic characteristics of nurses job satisfaction,occupationalcom mitment,leaving intend usingt test,analysis of variance,nurses job sa tisfaction before and after the intervention,occupational commitment and turnover will score were statistically analyzed,P<0.05 was statistic ally significant.Results1.Before the intervention,325 valid questionnaires were collected.The total score of job satisfaction was 98.4±17.7,the total score of occupational commitment was 78.1±14.1,the total score of turnover intention was 14.7±3.7,and the average score of turnover intention was2.4±0.6.2.After the intervention,a total of 299 valid questionnaires were collected because 26 subjects took more than 2 months' leave during the study.The total score of nurses' job satisfaction was 104.8±19.8,the total score of occupational commitment was 82.3±14.3,the total score of turnover intention was 14.0±3.9,and the total average score of turnover intention was 2.3±0.6.3.After the intervention,the total score of nurses' job satisfaction and scores in each dimension were higher than before the intervention,and the difference was statistically significant(P<0.05).The total score of occupational commitment,normative commitment dimension,economic cost commitment dimension,emotional commitment dimension and emotional cost commitment dimension were higher than those before the intervention,with statistically significant differences(P<0.05),while the difference of opportunity commitment dimension was not statistically significant(P>0.05).The total score of dimission intention was lower than that before the intervention,and the difference was statistically significant(P<0.05).ConclustionsIn the medical alliance mode,the main unit of the group provides nursing assistance to county-level hospitals,which can improve nurses' job satisfaction and occupational commitment,reduce the willingness of nurses to quit,and maintain the stability of nursing teams.
