ObjectiveTo explore the effect of case management based on the transtheoretical model of change on the quality of life,medication compliance,and risk factors of coronary heart disease in patients with coronary heart disease.MethodsA total of 104 patients with coronary heart disease were enrolled from March 2019 to November 2019 in the cardiac rehabilitation center of a top three hospital in Hunan Province by a type of experimental study.According to the inclusion criteria and exclusion criteria,104 patients were selected as subjects.The subjects were randomly divided into intervention group and control group,52 patients in control group,52 patients in intervention group.Control group implemented conventional discharge guidance,intervention group implemented TTM case management and the intervention tme last for three months.Before and after the intervention,relevant outcome indicators of coronary heart disease patients were collected for comparison,and the effects of TTM case management on quality of life,medication compliance,and risk factors of coronary heart disease were analyzed and evaluated.The data were analyzed by SPSS 19.0.The demographic data of the two samples were described by percentage(%)and frequency(n).Quality of life score and medication compliance score of the two samples were measured by t test.The number of drug compliance and risk factors of coronary heart disease in the two groups were tested Chi-test,the difference was statistically significant(P?0.05).Results(1)There is no statistical comparison of baseline data on the age,gender,marital status,education level,occupation,monthly family income,hospitalization payment method,diagnosis,whether or not to install PCI,whether or not have diabetes,whether or not have hypertension,smoking,drinking(P>0.05),so the baseline data of the two samples are comparable.(2)Before intervention,there was no statistically signifycant difference between the two groups in the total scores of quality of life,scorces of each dimension of quality of life,the scores of medication compliance,the numbers of medication compliance and risk factors of coronary heart disease(P>0.05).(3)After the intervention,the total score of life quality of the intervention group was higher than that the control group,and the difference was statistically significant(P<0.001),and the comparison of the scores of the dimensions of quality of life between the two groups showed that there was no statistically significant difference between the two dimensions of role physical and body pain(P>0.05),and the dimensions of physical functioning,general health,vitality,social functioning,mental health and role emotional of the intervention group was significantly higher than the control group(P<0.05).(4)After the intervention,the comparison of the score and the numbers of medication compliance of the intervention group was significantly higher than the control group,and the difference was statistically significant(P<0.001).(5)After the intervention,the comparison of two groups of risk factors for coronary heart disease showed that the number of blood pressure compliance,the number of LDL compliance,the number of fine heart rate compliance,and the number of sports compliance were higher in the intervention group than in the control group,the difference was statistically significant(P<0.05),and there were no significant differences in fasting blood glucose,smoking cessation,BMI,and waist circumference in two groups(P>0.05).Conclusion(1)The implementation of TTM case management can improve the quality of life of patients with coronary heart disease.(2)The implementation of TTM case management can improve medication compliance of patients with coronary heart disease.(3)The implementation of TTM case management reduce risk factors of coronary heart disease,but the effects of patients with coronary heart disease on fasting blood glucose,smoking cessation,BMI,and waist circumference in a short period of time need to be further explored.
