Objective The aim of this study was to explore the correlation between the levels of plasma homocysteine and the characteristics of coronary artery lesions in patients with premature coronary artery disease and to provide theoretical guidance for the diagnosis and treatment of the disease. Methods A total of 209 patients diagnosed as Premature coronary artery disease(PCAD)by coronary angiography(CAG)in the Emergency Department and the Emergency Intensive Care Unit of the Second Hospital of Anhui Medical University from December 2017 to June 2019 were selected for data collection. The diagnosis standard is that the lumen stenosis with at least one coronary artery or main branch ? 50%,with the age of male patients less than or equal to 55 years old and female patients less than or equal to 65 years old, which conforms to the CAD classification standard formulated by the New York Heart Association.According to the results of CAG,all the patients were divided into single vessel lesion group(n = 76),double vessel lesion group(n = 80)and three vessel lesion group(n = 53).Meanwhile,another 100 patients with late CAD were selected as the control group. The number of coronary lesions and the degree of coronary artery stenosis were recorded and the syntax score was performed,so as to compare the general clinical data, homocysteine, the number of branches involved in coronary lesions and the degree of stenosis, and to analyze the relationship between the level of plasma Hcy and the degree of coronary lesions. Results 1.According to the data collected, there were 150 male patients and 59 female patients in the premature CAD group. There were 64 male patients and 36 female patients in the late CAD group.SPSS16.0 software statistical analysis found that compared with the late CAD group, there were more male patients and higher body mass index(BMI)in the premature CAD group(single vessel disease group, double vessel disease group, three vessel disease group)with less proportion of patients with history of hypertension and diabetes. There was no significant difference in age, gender and BMI among the patients of single vessel disease group, double vessel disease group and three vessel disease group except for late CAD group(P > 0.05).2.The level of Hcy in patients with Premature CAD was significantly higher than that in patients with late CAD(20.95±10.06 vs.14.81±6.03,P <0.05).3.For patients with Premature CAD,the level of Hcy in three vessel disease group was higher than that in two vessel disease group and single vessel disease group(12.04±4.17 vs.18.87±6.43 vs.27.14±5.33,P <0.05),As an index to evaluate the severity of coronary artery disease, the syntax score showed a linear correlation with the plasma Hcy level in patients with Premature CAD(r=0.81,P<0.05).Conclusion1.The level of plasma Hcy was correlated with the Premature CAD.2.With the increase of Hcy level, the index of coronary artery involvement in patients with Premature CAD increased.3.Hcy level in patients with Premature CAD was positively correlated with the severity of coronary artery involvement, which had a certain predictive value.
