Objective:to investigate the changes of tear film and optical quality at different time after femtosecond laser in situ keratoplasty(FS-LASIK)with different cutting depth and the relationship between tear film by using the Optical Quality Analysis System(OQAS?).Methods:A total of 188 eyes were collected from 100 myopia after FS-LASIK surgery.According to the cutting depth of the cornea during surgery,it is divided into 1(cutting depth<60?m)?2(60?m?cutting depth<100?m)?3(cutting depth?100?m)groups.The tear film dynamic index(Mean OSI)and optical quality parameters(OSI?MTF-cutoff?SR?VA100%)were measured before operation,10 days after operation,1month after operation and 3month after operation respectively by OQAS?system. The data were analyzed by using IBM SPSS statistics 22.0 software.Using (?) ± s to describe the data.The differences of each index were compared by Kruskal-Wallis H test with several independent samples in non-parametric test,the spearman correlation analysis method was used to investigate the relationship between tear film(Mean OSI)and optical quality(OSI?MTF-cutoff?SR?VA100%).Results:1.In the first group,UCVA 10 d,1mo and 3mo after surgery showed no statistically significant difference compared with BCVA before surgery(H=1.044,P=0.791).In the second group,UCVA 10 d,1mo and 3mo after surgery showed no statistically significant difference compared with BCVA before surgery(H=0.748,P=0.862).UCVA of the third group 10 d,1mo and 3mo after surgery showed no statistically significant difference compared with BCVA before surgery(H=0.036,P=0.998).There was no significant difference in postoperative naked eye visual acuity among the three groups(H=3.224,P=0.200).There was no significant difference in postoperative visual acuity between the three groups(H=3.437,P=0.179).There was no significant difference in postoperative visual acuity between the three groups of 3mo(H=1.972,P=0.373).2.Changes in tear film and optical quality:(1)Mean OSI in the first group was higher than that before the operation at 10 d after the operation(P<0.05),and there was no significant difference between 1mo and 3mo after the operation and that before the operation(all P>0.05).OSI 10 d and 1mo after operation were higher than before operation(all P<0.05),while there was no difference between 3mo and before operation(P>0.05).MTFcutoff,10 d after SR and1 mo were lower than before surgery(all P<0.05),and there was no significant difference between 3mo and before surgery(P>0.05).There was a difference between VA100% preoperatively and postoperatively(P<0.05),but there was no significant difference between VA100% preoperatively and postoperatively(P<0.05).(2)In the second group,Mean OSI,OSI 10 d,1mo and 3mo were all higher than those before surgery(all P<0.05).MTFcutoff,va100%,and SR were lower than those before the operation 10 d,1mo,and 3mo after the operation(all P<0.05).(3)In the third group,Mean OSI was higher than before(all P<0.05)at 10 d and 1mo after surgery,while there was no difference between 3mo and before surgery(P>0.05).10 d after OSI was higher than that before surgery,and the difference between 1mo and 3mo after surgery was not statistically significant(P>0.05).MTFcutoff and va100%were lower than that before surgery at 10 d after surgery(all P<0.05),and the difference between 1mo and 3mo after surgery was not statistically significant(all P>0.05).At 10 d after SR,1mo was lower than before SR(all P<0.05),and there was no difference between 3mo and before SR(P>0.05).3.Correlation between tear film and visual quality: Mean OSI and OSI were positively correlated at fs-lasik preoperatively and 10 d postoperatively,1mo postoperatively and3 mo postoperatively(all P<0.05).Mean OSI was negatively correlated with mtf-cutoff,SR and va100% before and 10 d after fs-lasik,1mo and 3mo(all P<0.05).Conclusion:1.The stability of tear film decreased in the early stage after FS-LASIK,and gradually became stable for a period of time after operation;2.Visual quality will decline in a short period of time after FS-LASIK,and then gradually recover.About three months after surgery,all indicators will basically return to the preoperative level.3.The stability of tear film after FS-LASIK was synchronized with the change of visual quality.The more stable the tear film was,the better the visual quality was,which was positively correlated.
