The Influence Of Overnight Orthokeratology On Tear Film

Objective:To study the effect of overnight orthokeratology on tear film with keratograph, and to analyze the variation trend and figure out the correlation of tear meniscus heights frist tear break-up time、average tear break-up time.Methods:The juvenile myopia paitients were selected in Shen Yang He Eye Hosptial with overnight orthokeratology (OK) lens from june 2014 to January 2015.Thirty teenagers were recruited (males,13; females,17). The average was 12.5±2.5 years (range,8 to 18 years), the Spherical Equivalent refraction was -1.25 to -6.00D, the average Spherical Equivalent refraction was -3.40±1.10D. Uncorrected visual acuity, slit-lamp, DR-1 tear interferometry and keratograph were examined. Subjects were evaluated at baselline,1 week、2 weeks、4 weeks and 12 weeks after wearing overnight orthokeratology lens. After treatment, frist non-invasive keratograph tear break-up time(NITBUTf)、average non-invasive keratograph tear break -up time(NITBUTav)、non-invasive keratograph tear meniscus height (NIKTMH) were analyzed by one-way analysis of variance (AN OVA).Results:NITBUTf were (8.62±4.45s)、(11.02±5.72s) 1 day and 1 week after wearing overnight orthokeratology lens, showing remarkable decline in comparison with (14.64±5.51s) of before wearing orthokeratology lens (p<0.05,for all), however, there was no significant difference in each other(p=0.319). NITBUTf was 1 day after wearing orthokeratology lens, showing significant decrease in comparison with 2 weeks (13.08+5.83s)> 4 weeks (13.32±5.73s)、12 weeeks (13.32±5.45s) after wearing orthokeratology lens (p<0.05). NITBUTf was 1 week after orthokeratology, showing decrease in comparison with (13.08±5.83s)、(13.32±5.73 s) 2 weeks and 4 weeks after orthokeratology,but there were no significant variation (p> 0.05). There were no significant variation in NITBUTf among 1 week、2 weeks、4 weeks、12 weeks after wearing orthokeratology lens (p> 0.05,for all).②NITBUTav were (11.34±4.79s)、(13.19±5.60s) 1 day and 1 week after wearing orthokeratology lens, showing remarkable decline in comparison with (16.59±5.31s) of before wearing orthokeratology lens (p<0.05,for all), however,no significant difference was seen in each other(p=1.000). NITBUTav was 1 day after wearing orthokeratology lens, showing significant decrease in comparison with 2 weeks (15.19±5.67s)、4 weeks (14.40±5.43s)、12 weeeks (15.76±5.23s) after wearing orthokeratology lens (p<0.05). There were no significant variation in NITBUTf among 1 week、2 weeks、4 weeks、12 weeks after wearing orthokeratology lens (p>0.05,for all).③Tear meniscus height was 1 day (0.26±0.07mm) after wearing orthokeratology lens, showing significantly increased in comparison with 1 week (0.23±0.04) mm、2 weeks(0.22±.03mm)、4 weeks(0.22±0.03mm)、12 weeks(0.21±0.03mm) of after wearing orthokeratology lens, and (0.23±0.02mm) of before wearing orthokeratology lens (p<0.05,for all),TMH were 2 weeks and 4 weeks after wearing orthokeratology lens, showing no significant decline in comparison with before wearing orthokeratology lens (p>0.05),TMH was 12 weeks after wearing orthokeratology lens, showing significant decline in comparison with before wearing orthokeratology lens and lweek after wearing orthokeratology lens (p<0.05).④Spearman correlations were performed to analyze the relationship between NITBUTf and NITBUTav (r=0.944), NITBUTf and tear meniscus height (r=0.025),NITBUTav and tear meniscus height(r=0.031)Conclusions:It is in the frist week when Wearing overnight orthokeratology lens affects the tear film functional, after 1 week,the tear film function had tendency to restore the state before wearing orthokeratology lens. It is a significant positive correlation between NITBUTf and NITBUTav,in addition,they are weak correlation with NITMH. Keratograph is a noninvasive、objective approach to exam tear film functional,which has offered a new way for evaluating the stability of tear film.
