Objective To estimate the association prenatal BPA exposure and prenatal BMI with child neurobehavior at(13.18±5.46)month of age in a prospective cohort of 931 mothers and their children.Methods The Ma'anshan Birth Cohort Study,one of the sub-cohorts of the China-Anhui Birth Cohort Study,was used as a platform to recruit pregnant women and their offspring for the study,and the Maternal and Child Health Record Form during pregnancy and childbirth was used to investigate the basic situation of pregnant women.The concentration of free BPA in the serum of mothers at 12.10±3.83 weeks of gestation was determined by solid phase extraction with internal isotope labeling and high performance liquid chromatography tandem mass spectrometry.Prior to testing,the frozen serum was gradient thawed,i.e.,first from-80? to-20? in the refrigerator for several hours,then from-20? to 4? in the refrigerator for several hours before being taken to room temperature for thawing.The preconception BMI was investigated,and the follow-up BMI during pregnancy was investigated.At the age of infants and young children(13.18 ± 5.46),0 ? 6-year-old pediatric examination table of neuropsychological development(pediatric neuropsychological table for short)was used to perform neurobehavioral development in children from April to October2011.The 931 mother-child pairs finally included were taken as the research object.The prenatal serum BPA concentrations were divided into three groups according to theinterquartile range of the percentile,low exposure group(?0.1 213 ng/ml),medium exposure group(0.1 214 to 0.3 963 ng/ml)and high exposure group(>0.3 963 ng/ml).The association between prenatal BPA exposure and offspring neurobehavioral development was examined using a generalized infant neurobehavioral developmental quotient as a dependent variable.Secondly,the association between prenatal BMI and neurobehavioral development of offspring was examined using a generalized linear model with the independent variable of prenatal BMI(low BMI group as reference).Finally,after the prenatal BMI grouping and prenatal BPA exposure levels were recombined as independent variables,the generalized linear model was used to test the association with the neurobehavioral development of offspring.Results The age of 931 infants and children ranged from 2.97 to 28.16(13.18±5.46)months and 498 boys(53.5).The overall BPA detection rate was 87.5%,with a higher BPA detection rate of 92.6% among mothers aged 30 years and over.The average scores of development quotient,fine movement,adaptive ability,language ability and social ability were 97.88 ± 16.32,97.16 ± 15.35,99.64 ± 15.47,95.3 ± 16.04 and98.95 ± 14.76 respectively.There were 646 in the high BMI group and 284 in the low BMI group.Serum BPA concentrations were grouped by interquartile range of the percentile.Using the low-exposure group as the reference level,adjusting for potential confounding factors(sex,mode of delivery,feeding at 4 months of age,monthly income per capita,mother's level of education,age at month,prematurity),and after age stratification,the effect of high BPA exposure on neurobehavioral development in infants and young children was found only at 12 to 18 months of age [language: ?(95%CI):-6.656(-13.058,-0.253);so CIal behavior: ?(95% CI):-7.401(-12.972,-1.830)].Using the latent variable mixed growth model,the trajectory of prenatal BMI change was divided into low BMI group and high BMI group,and the asso CIation of high BMIgroup with neurobehavioral development of offspring was not statistically significant(P>0.05).The prenatal BMI and BPA exposures were divided into 6 groups(high BMI with high BPA,high BMI with high BPA,high BMI with low BPA,low BMI with high BPA,low BMI with low BPA,low BMI with low BPA)and the effects of each group on infant neurodevelopers were analyzed using generalized linear models,with the prenatal high BMI with high BPA exposure group having an effect on infant gross motor [?(95% CI):-3.879(-7.71,-0.047)],adaptive ability [?(95% CI):-4.191(-7.88,-0.501)],so CIal behavior developer [?(95% CI):-3.861(-7.374,-0.349)].Conclusion High-level prenatal serum BPA concentration was adversely associated with social development in infant,and 12-18 months of age may be the detection window of the correlation effect.Although the association effect between prenatal BMI and neurobehavioral development in infants was not statistically significant,when maternal prenatal BMI was high,moderate concentrations of BPA exposure could cause changes in neurobehavioral development in offspring,suggesting that maternal BMI levels should also be concerned when studying the relationship between maternal BPA and offspring outcomes.
