Objective: To investigate the present situation of the life quality of patients and their influencing factors in one year after laparoscopic radical gastrectomy in Yan'an city,and to provide a basis for clinical targeted nursing measures to further improve the quality of life of patients.Methods:178 patients who underwent laparoscopic radical gastrectomy at Yan'an university affiliated hospital and other 3A-grade hospitalsafter one year.And the 178 patients who met the exclusion criteria were examined by the department of gastrointestinal surgery or oncology in the Yan' an University affiliated hospital from January 2019 to January 2020.The research object was selected by convenient sampling method.The survey included general data survey,medical response questionnaire(MCMQ),EORTC quality of life measurement scale QLQ-C30 and QLQ-STO22 questionnaire.The status quo of patients' quality of life,the status quo of medical coping methods and the medical norm were compared with one year after operation.At the same time,single factor and multiple linear regression analysis were used to identify the influencing factors affectingthe quality of life of the patients.Results:(1)The present situation of the life quality:(1)The results of QLQ-C30 scale demonstrated that the overall health score of the patients in one year after laparoscopic radical gastrectomy were 68.3±5.67 lower than the norm;the functional field scores were lower than the norm;the fatigue score in the symptom field was higher than the norm;the scores of economic difficulties,loss of appetite and insomnia in the single item were higher than the norm.(2)The results of QLQ-STO22 scale showed that the scores of reflux,pain,eating disorder,taste change and body shape were higher than those of healthy patients(P<0.05)one year after laparoscopic radical gastrectomy.(2)The scores of each dimension of medical coping style of patients one year after laparoscopic radical gastrectomy were as follows: face coping 22.47±7.79,flexion coping10.33±3.11,avoidance coping16.21±3.01,except avoidance coping,face coping and yieldcoping scores were significantly higher than norm(P<0.01).(3)According to the results of single factor analysis,there were many influencing factors in each dimension of quality of life of the patients,among which sex,age,payment method of medical expenses,main caregivers,surgical options and pathological stage of disease on the overall health condition were statistically significant.(4)Pearson correlation analysis: there was no significant correlation between coping and overall health level,yield coping was negatively correlated with overall health level(P<0.01),and avoidance coping was positively correlated with overall health level(P<0.01).(5)Multivariate linear regression analysis: the quality of life of the patients was affected by a variety of factors,such assex,age,payment method of medical expenses,main caregivers,operation mode,pathological stage,yield response and avoidance response(P<0.05).Conclusion:(1)The quality of life of the patients were at a low level one year after laparoscopic radical gastrectomy.(2)Avoidance as a positive coping style affects the quality of life of patients.(3)There are many factors affecting the quality of life of patients.Andthe main factors are pathological stage,operation mode,main carer and payment of medical expenses.
