Prediction Of Cognitive Impairment In Cerebral Small Vessel Disease(CSVD) By Cranial Magnetic Resonance Imaging(MRI) Score And The Level Of Serum Inflammatory Factors

Objective: To investigate the the relationship between general information,inflammatory factors IL-6,IL-10,CRP and chemokines MCP-1,CX3CL1 in peripheral blood,and cranial magnetic resonance imaging score and the occurrence of cognitive impairment in cerebral small vessel disease.Methods: Patients with cerebral small vessel disease who were admitted to the department of neurology,Shengjing Hospital of China medical university from January2019 to April 2019 and met the inclusion criteria were selected.Then,general information and cranial imaging results were collected,the MoCA scores were improved,blood samples were collected,and the levels of inflammatory factors IL-6,IL-10,CRP and chemokines MCP-1,CX3CL1 in serum were detected by using ELISA method.Results: 1.A total of 74 cases with cerebral small vessel disease,with an average age of67.59±7.65 years old.Female patients account for the majority,and male patients account for only 31.08%.The percentage of the vascular risk factors are hypertension(55.41%),smoking history(27.03%),drinking history(27.03%),coronary heart disease(24.32%),diabetes(22.97%).The test results of blood lipid are as follows: triglyceride increases by 27.03%,total cholesterol increases by 9.46%,and LDL increases by 40.54%.According to MoCA score,the cases are divided into the cognitive impairment group(36cases)and the non-cognitive impairment group(38 cases).There is no significant difference in the distribution of the above information between the two groups.The cranial MRI performance of patients with cerebral small vessel disease are evaluated by using the total SVD score and Fazekas score,with no statistically significance difference between the cognitive impairment group and the non-cognitive impairment group(P <0.01).Age,gender,vascular risk factors,test index of blood lipid,indexes of IL-10,IL-6,MCP-1,CRP andCX3CL-1 are adjusted for cognitive impairment in patients with the cognitive impairment in patients with cerebral small vessel disease as the dependent variable.Besides,both the total SVD score [Odds Ratio(OR)=5.036,95% CI:1.999-12.686,P<0.01] and Fazekas score [Odds Ratio(OR)=1.906,95% CI: 1.163-3.156,P<0.01] are risk factors for cognitive impairment in cerebral small vessel disease.3.ROC curves are drawn with the Fazakes score(sensitivity 69.4%,specificity 86.8%,AUC=0.834)and the total SVD score(sensitivity 88.9%,specificity 77.9%,AUC=0.904)as references,respectively,showing that the diagnostic efficacy of the total SVD score is better than that of the Fazakes score.4.Between the cognitive impairment group and the non-cognitive impairment group,there is no statistically significant difference in the levels of chemokines CX3CL-1,MCP-1 and inflammatory factors CRP,IL-6,while there is a statistically significant difference in the levels of IL-10 between the groups(T=-2.119,P=0.043).Moreover,the analysis of the univariate logistic regression model shows that the significant difference is not statistically significant.Conclusion: There is no significant correlation between cognitive impairment and age,gender,smoking history,drinking history,hypertension,diabetes,and high fat level.There is a significant correlation between the total SVD score and the Fazekas score of cranial imaging and the occurrence of cognitive impairment in cerebral small vessel disease,with the diagnostic efficacy of the total SVD score superior to that of the Fazekas score.Besides,the IL-10 is involved in the occurrence and development of cognitive impairment in cerebral small vessel disease with a not clear effect.
