Objective:To investigate the correlation between total cerebral small vessel disease MRI score and overall cognitive functions and different cognitive domains.Methods:We collected the clinical data of patients who were admitted in neurolo gy department for treatment in the XXX Hospital and received 3.0 MRI(T1-weig htd imaging,T2-weighted imaging,fluid-attenuated inversion recovery sequence an d susceptibility-weighted imaging)from September 2018 to December 2019,and t hey were assessed on the neuropaychological scales.We used the total burden of CSVD to evaluate the four neuroimaging feateres that included lacunar infarction,white matter hyperintensity,cerebral microbleeds and enlarged perivascular space.Neuropsychological scales were used to evaluate overall cognitive function,imme diate memory,short-term delayed memory,long-term delayed memory,re-recognit ion ability,information processing speed,reasoning and conversion ability and att ention in the CSVD patiets: MMSE and MOCA scale were used to evaluate ove rall cognitive function of patients,Rey auditory verbal learning test were used to evaluate the immediate memory,short-term delayed memory,long-term delayed memory and re-recognition ability,trail making test and Stroop color-word test w ere used to evaluate the executive function which including intormation processin g speed,reasoning and conversion ability and inhibition of interference,digit spa n test was used to evaluate the patients' attention,clock drawing test was used t o evaluate the patients' visual spatial function.And we did the statistical analysi s.Results:(1).Evaluation of Cognitive Function in CSVD patients: The CSVD pati ents' overall cognitive function(MMSE,MOCA),long-term delayed memory,re-reco gnition ability scores were significantly lower than the control group(P<0.05),an d the time needed to complete the information processing speed and reasoning a nd conversion ability and inhibition of interference tests were significantly longer than the control group(P<0.05).There was no significant difference in immedia te memory,short-term delayed memory,reasoning and conversion ability,attention sc ores between the two groups(P>0.05).(2).The people in different groups of MRI burden score have different demaged cognitive domains: The MOCA score of MRI burden score 2,3,4 groups was significantly lower than than the control group(P < 0.05).The MMSE score and re-recognition ability score of MRI burden score 3,4 groups were significantly lower than the control group(P<0.05).The information processing speed of MRI burden score 3 group was significantly prolonged when compared with the control group(P<0.05).The immediate memory,long-term delayed memory,inhibition of interference ability of MRI burden score 4 group were significantly lower than the control group(P<0.05).(3).The correlation between total cerebral small vessel disease MRI score and cognitive function: The total cerebral small vessel disease MRI score was negatively correlated with MMSE,MOCA,short-term delayed memory,long-term delayed memory and re-recognition ability scores(r =-0.312 ~-0.571,all P < 0.05),and was positively correlated with the information processing speed(r=0.420,P <0.05).There was no significantly correlation between total cerebral small vessel disease MRI score and reasoning and conversion ability,inhibition of interference ability,attention,visual spatial function(P>0.05).Conclusion:1.The total cerebral small vessel disease MRI score is associated wit h cognitive decline especially in the overall cognition,executive function(informat ion processing speed)and memory(short-term delayed memory,long-term delaye d memory,re-recognition ability).2.The people of different total cerebral small v essel disease MRI score have different cognitive dysfunction: When the total cere bral small vessel disease MRI score increased,the cognitive dysfunction domains were inceresed too.3.In the assessment,the MOCA scale has a higher sensitivity than the MMSE scale of the overall cognitive impairment.
