Object:The purpose of this study was to evaluate the clinical efficacy of acupoint catgut embedding in the treatment of vertebral artery cervical spondylosis through clinical randomized controlled trials,and to observe the effect of acupoint catgut embedding on the serum tumor necrosis factor-?(TNF-?)in patients,to explore the possible mechanism of this therapy in the treatment of vertebral artery cervical spondylopathy,and to provide a scientific basis for the treatment of vertebral artery cervical spondylosis.Method:In this study,60 patients who met the criteria were randomly divided into treatment and control groups.In the treatment group,bilateral Fengchi,Cervical Jia Ji,Tianzhu,Jian Jing,Houxi,Hegu,and Waiguan were used for acupoint embedding therapy,once every 10 days,the treatment is a course of treatment,a total of 2 courses of treatment.The control group selected the same acupuncture points as the treatment group for routine acupuncture treatment,once a day,continuous treatment for 6 days,and rest for 1 day as a course of treatment,a total of 3 courses of treatment.Observe the cervical dizziness symptom and function evaluation scale(ESCV)score,transcranial Doppler(TCD)test,blood index TNF-?,total effective rate and recurrence rate of the two groups of patients before and after completion of the group Changes after treatment.All the data in the study were processed using SPSS22.0 statistical software to analyze the relevant data.Result:1.The baseline data of the two groups of patients before treatment: The patient's age,sex,course of disease,each score and total score of ESCV,TCD test results,and serum TNF-? content were all obtained by statistical analysis.The two groups of patients had the same baseline before treatment,and the difference was not statistically significant(P>0.05),which can be compared.2.Each score and total score of ESCV: After treatment,the scores of the two groups of patients were significantly improved(P<0.01);Compared between the two groups,there was a statistically significant difference in psychological and social adaptability and total score(P<0.01);There were statistical differences in dizziness,neck and shoulder pain,headache,daily life and work ability(P<0.05),indicating that the symptoms of both groups improved after treatment.But the improvement in the treatment group was significantly better than the control group.3.Comparison of TCD test results: After treatment,the vertebral-basal artery blood flow rate of the two groups of patients was significantly improved(P<0.01),and there was a statistical difference between the two groups(P<0.05).It shows that both treatment methods can improve the vertebral-basal artery blood flow rate,but the treatment group is better than the control group.4.Comparison of serum TNF-? content: After treatment,the serum TNF-? content of the two groups was significantly improved(P<0.01),and there was a statistically significant difference between the two groups(P<0.01).It shows that both treatment methods can improve the content of TNF-? in the serum of patients,but the improvement in the treatment group is more obvious.5.After treatment,the total effective rate of the treatment group was 93.33%,while the total effective rate of the control group was 83.33%,indicating that both treatment methods were effective,and the overall efficacy of the treatment group was higher than that of the control group(P<0.05).6.The follow-up results showed that the relapse rate of the treatment group was 13.33%;the relapse rate of the control group was 36.67%,and the relapse rate of the treatment group was significantly lower than that of the control group(P<0.05).This indicates that the long-term curative effect of acupoint catgut embedding on vertebral artery cervical spondylopathy is not easy to recur.Conclusion:1.Catgut embedding at the acupoints and conventional acupuncture can effectively treat cervical spondylosis of the vertebral artery type,both can improve the patient's symptoms and blood supply to the vertebral-basal artery,and reduce the content of TNF-? in the serum.2.The overall clinical efficacy of acupoint catgut embedding in the treatment of cervical spondylosis of vertebral artery type is superior to conventional acupuncture,and the long-term efficacy is more advantageous.3.The mechanism of acupoint catgut embedding therapy for cervical spondylosis of vertebral artery type may be related to lowering the content of TNF-? in the serum,thereby reducing the sterile inflammation around the soft tissue,improving the damage and adhesion status of the soft tissue,and improving the blood supply of the vertebral artery.
