Clinic Retrospective Analysis Of 84 Cases Of Chronic Kidney Disease With Syndrome Of Yin Injured By Damp-heat Treated By Ganlu Huacai Decoction

Objective: This study retrospectively summarized the clinical datas of 84 patients with syndrome of yin injured by damp-heat in CKD treated with Ganlu huacai decoction,and it analyzed the clinical efficacy and the rules of TCM property by Ganlu huacai decoction on syndrome of yin injured by damp-heat in CKD,Which provides a clinical basis and evidence for the treatment of CKD by TCM.Method: A retrospective clinical analysis was used to screen 84 in-patients who were admitted to the 8th Department of Nephrology of Heilongjiang Traditional Chin ese Medicine Hospital from January 1st,2016 to December 30 th,2019 and met the i nclusion criteria.Stages of CKD,gender,age,course of disease and other general da ta,renal function and other curative indicators and Chinese medicine addition and su btraction are recorded.Observe the total curative effect,TCM syndrome curative effe ct,TCM syndrome score and main clinical symptom score before and after treatment and summarize the rules of TCM property.Results: 1.General information:(1)Among all the CKD hospitalized patients in the 8th Department of Nephrology from2016 to 2019,the syndrome of yin injured by damp-heat accounted for 11.03%;(2)Among the 84 collected cases,The number those in the age group of 51-60 years is the highest,accounting for 30.95%;the number of cases in CKD5 is the largest,accounting for 75.00%;the number of cases with protopathy of chronic glomerulonephritis is the largest,accounting for 51.19%.2.Therapeutic evaluation:(1)Among the 84 cases,the total effective rate was 86.90%;(2)Among the 84 cases,the TCM syndrome effective rate was 89.28%,The effective time of 75 effective cases was concentrated in 2 weeks to 4 weeks,accounting for about69.33%;after the treatment,total score of TCM syndromes and Main clinical symptoms was significantly lower than before,with significant statistical difference(P<0.01);(3)Renal function: after the treatment,the levels of Scr,BUN,UA were significantly lower than before,with significant statistical difference(P<0.01);the level of e GFR wassignificantly higher than before,with significant statistical difference(P<0.01).3.Analysis of prescription medication:(1)In this study,the patients used a total of 146 medicinal flavors,of which,in addition to the basic medicine of the Ganlu huacai detection,the rest of the medicines used had a total of 134 flavors,and the total number of TCM used more than 10 times was 35 flavors;frequency of use The highest 10 kinds of Chinese medicines are: fruit of caoguo,liquorice root,barbary wolfberry fruit,weeping forsythia capsule,peony root,largehead atractylodes rhizome,morinda root,indian bread,pinellia tuber,tangshen;(2)Among the 146 medicinal flavors: cold medicines(41.78%)accounted for the most among the four nature of drugs,followed by warm medicines(36.30%),and heat medicines(2.05%)at the lowest;the top three of the five flavors were the sweet medicines(32.05 %),bitter medicines(31.20%),pungent medicines(23.93%);the top three of channel tropism are liver channel(20.78%),kidney channel(18.18%),lung channel(13.51%);The five categories of TCM with the largest proportions are: heat-clearing drugs(18.49%),damp-clearing drugs(12.33%),superficies-relieving drugs(7.53%),blood-activating drugs(6.85%),and yin-supplementing drugs(6.85%).Conclusion:1.The patients of CKD with syndrome of yin injured by damp-heat is mainly in the elderly,the patients of syndrome of yin injured by damp-heat is mainly in CKD5.2.Ganlu jiajian decoction can significantly improve the clinical symptoms of patients of yin injured by damp-heat in CKD,such as nausea and vomiting,dry mouth and pharynx.3.Ganlu jiajian decoction can significantly reduce serum Scr,BUN and UA levels in patients of yin injured by damp-heat in CKD,improve glomerular filtration rate andprotect residual renal function.4.Ganlu huacai decoction's prescription is mainly based on sweet and cold,mostly into liver,kidney and lung channel,and the traditional Chinese medicines for treating syndrome of yin injured by damp-heat in CKD mainly are heat-clearing drugs,damp-clearing drugs,superficies-relieving drugs,blood-activating drugs,and yin-supplementing drugs.
