BackgroundStroke,also known as cerebrovascular accident(CVA),is the third leading cause of death after heart disease and cancer in the world.It has the characteristics of high prevalence,high mortality rate,high disability rate and high recurrence rate China Stroke Prevention and Control Report 2018 shows that the proportion of cerebrovascular disease in the death of disease in China's residents in 2017 is 23.18%in the rural population and 20.52%in the urban population,which means that at least one in every five deaths dies of stroke.It is also the main cause of long-term disability,with 80%of stroke patients returning to the community and family with varying degrees of disability to continue their rehabilitation.At present,about 3/4 of stroke survivors have varying degrees of loss of ability to work and live,need the care of others,and bring a heavy economic burden to the family and society.Foreign countries define family caregivers as "people who live with patients and are mainly responsible for the family care of patients ",generally for the spouse,children,daughter-in-law/son-in-law of patients,who are the pillars of service to stroke survivors and play a vital role in the recovery of stroke patients.family members who are the main caregivers in the long-term care process,under the physical,psychological,work,social,economic,and family relations and other aspects of pressure,heavy care burden has seriously affected the quality of life of caregivers,while at present,less attention is paid to the quality of life and social support of family caregivers in patients with stroke sequelae,more attention so pay on it.ObjectivesTaking the health-related quality of life measurement scale as a tool,this paper explores the main factors affecting the quality of life of family caregivers in patients with stroke sequelae,analyzes the correlation between the quality of life and social support of family caregivers in patients with stroke sequelae,and puts forward relevant measures and suggestions to improve the quality of life of family caregivers in patients with stroke sequelae.Methods1.Literature AnalysisThrough extensive retrieval,reading and bibliometric analysis of the relevant literature of stroke caregivers,this paper summarizes the research status,research hot spots and development trends of stroke caregivers in recent years,puts forward the hypothesis of this study,establishes the model of influencing factors,and designs the questionnaire.2.Questionnaire Surveyrandom distribution of questionnaires on site in a large third-class general hospital from April 2019 to August 2019.The contents of the survey included:general condition of patients,ability of daily activities;general demographic factors of caregivers,care situation,family situation,quality of life of caregivers and social support.In this study,265 questionnaires were distributed,242 valid questionnaires were recovered,and the effective recovery rate was 91.32%.3.Empirical AnalysisThe data input was carried out by EXCEL,and the influencing factors model of this study hypothesis was statistically analyzed by SPSS20.0,Statistical methods include descriptive analysis,reliability and validity analysis,single factor analysis,correlation analysis,multiple linear regression analysis,and finally the quality of life and social support status of family caregivers of stroke sequelae were obtained,as well as the related influencing factors.4.Interview Analysisrespondents may be affected by patients,other caregivers and other external environment during the course of the survey.this study adopted a one-to-half structured interview with caregivers to strive for the most authentic and accurate feelings of caregivers.Results1.A total of 265 questionnaires were sent out in 1.,255 questionnaires were recovered,13 invalid questionnaires were eliminated,and 242 valid questionnaires were recovered,with an effective rate of 91.32%2.most of the family caregivers of stroke sequelae investigated were women(58.70%),with an average age of 53.76±16.60 years,with the patient's spouse(50.80%)or children(36%)predominating.more than 80%of caregivers had at least one chronic disease and 68.60%of caregivers indicated that the care burden was heavy or very heavy.3.Results of reliability and validity of the 3.quality of life measurement scale:the total Cronbach's a coefficient of the scale was 0.906,the Cronbach's a coefficient of the two subscales of physiology and psychology was 0.892 and 0.890,respectively,and the scores of each dimension were greater than 0.7;the KMO statistic was 0.904,Barllet the spherical test result was 5951.769,and the cumulative variance contribution rate was 70.127%.4.total score of quality of life of family caregivers in 242 patients with stroke sequelae in this survey was 68.47±15.59,the total score of physiological subscale was 71.29 and 16.74,and the total score of psychological subscale was 65.75±17.90,all of which were lower than that of ordinary urban residents in Guangdong Province.The total score of social support was 38.81±7.05,the score of subjective support was 9.15±2.42,the score of objective support was 22.79±5.0,and the score of support utilization was 6.87±1.83,which was generally at the middle and low level.5.multivariate analysis results showed that five factors,such as length of sleep per day,family age,social support,family per capita monthly income,and relationship with patients,were the most important factors affecting the quality of life of family caregivers in patients with stroke sequelae.6.correlation analysis results showed significant positive correlation between the quality of life of family caregivers and social support dimensions in patients with stroke sequelae(P<0.05)Conclusion:1.SF-36 scale should be used to evaluate the quality of life of family caregivers in patients with stroke sequelae.2.Those who are over 69 years of age and live in urban communities with moderate dysfunction,the carers themselves are older,less educated,live in rural areas,have a heavy burden of self-conscious care,sleep<4 hours,take care of 12 hours>day,have lower per capita monthly income and poor quality of life for those who lack social support.3.The main factors affecting the quality of life of family caregivers in patients with stroke sequelae are:sleep time,caregiver age,social support,family per capita monthly income,and relationship with patients.4.There is a positive correlation between the quality of life and social support of family caregivers in patients with post-stroke sequelae.5.Three suggestions are put forward for the main factors affecting the quality of life of family caregivers in patients with stroke sequelae:first,it is necessary to raise the public's attention to this group;secondly,to guide them internally to adopt the correct coping style and encourage them to actively carry out self-health management;and thirdly,to provide as much support and help as possible from outside,thus helping to improve the quality of life of patients and caregivers.
