Objective:1.To explore the effectiveness and feasibility of the dynamic quantitative evaluation system in the treatment of keloid;2.Establish a reasonable dynamic quantitative evaluation system to provide new ideas for the standardized treatment of keloid.Research methods:Retrospective analysis was performed on 27 patients with keloid surgery in our department from April 2015 to December 2019,including a total of 45 patients with keloid.All patients received surgical resection combined with postoperative adjuvant radiotherapy,sustained wound tension-reduction fixation for more than 3 months,and oral tranilast treatment for more than 6 months.All patients were followed up in strict accordance with the follow-up procedures at regular intervals,and were followed up for at least one year.The patients were followed up once a month within one year after the surgery.The patients with higher scores were followed up more frequently until their scores were reduced.The follow-up interval was extended to 3 months for those with lower scores after 1 year,and to 6 months for those with lower scores after 2 years.The Vancouver Scar Scale(VSS)was used for evaluation before and during postoperative follow-up.Adjust treatment according to vancouver scale assessment score,blood vessels distribution,colour and lustre monomial index score > 2 points to fuel pulse laser interference,high single index score > 2 points,flexibility,march An Nai De injection treatment,the initial score is higher,have a relapse tendency of keloids,should increase the number of intervention,to score two points or less.HAMA Scale(HAMA)and self-rating Anxiety Scale(SAS)were used to evaluate the psychological condition of the patients,and intervention was provided when necessary.Finally,the final efficacy was evaluated with a 3-level classification system,and visual analogue Scale(VAS)was used as the index to measure the subjective satisfaction of the patients.Results:Of the 45 cases,34 were cured(76%),5 improved(11%),and 6 relapsed(13%).Vancouver score was 14.0(13.0,15.0)before treatment and 2.8(1.9,4.0)after treatment(P < 0.001),and the difference was statistically significant.The Vancouver score at different nodes of the patients was significantly lower than that before the operation.The Vancouver scores of keloids with a follow-up density of 2 and greater than 2 were significantly lower than those with a follow-up density of less than 2,and the difference was statistically significant(P< 0.001).There was no significant difference between the Vancouver scores of keloids with a follow-up density of 2 and greater than 2(P=0.268).VAS before and after treatment were compared,P<0.05,and the changes in VAS scores after treatment were statistically significant compared with those before treatment.Conclusion:1.The dynamic quantitative evaluation scheme has clinical feasibility in the clinical treatment of keloid,and the treatment scheme and follow-up time can be reasonably adjusted according to the score;2.As a systematic,normative and standardized assessment system,dynamic quantitative assessment provides new ideas for the standardized treatment of keloid,and reduces the recurrence rate of keloid through regular follow-up,timely intervention and other measures.
