Objective:To understand the malnutrition status of elderly inpatients in the community,and discuss the influencing factors of malnutrition in elderly inpatients and the correlation with elderly patients.Methods:1.One hundred and thirty elderly patients were recruited in the Department of Internal Medicine of Guandu Community Health Service Center from June 2017 to October 2017.Clinical data and laboratory examination outcome were enlisted,and according to the scale of Mini Nutritional Assessment short form(MNA-SF),the elderly patients were divided into patients with nutrition risk group and without nutrition risk group;2.The general demographic data of the patients(gender,age,education level,past medical history)was registered when the patients were involved;Height and weight were measured and body mass index(BMI)was calculated;hemoglobin and albumin were detected at same time;3.Comprehensive Geriatric Assessment(CGA),which are mainly including coexistence of multiple diseases,multiple medications,teeth,hearing,vision,urinary incontinence,chronic constipation,sleep,chronic pain,daily aassessment of living ability,cognitive function,and depression were evaluated by the doctors.All data were tabulated,and statistical analyses were performed between the two groups;4.Multiple linear regression analysis was applied to explore the influencing factors of malnutrition in elderly inpatients in the community.Results:1.43 patients out of 130 aged inpatients has high nutritional risk or malnutrition,and the incidence of malnutrition in aged inpatients was 33%.The statistic analysis were performed in two groups of patients with nutritional risk and without nutritional risk.The results showed that:although the proportion of elderly women with nutritional risk[55/81(67.9%)]was higher than that of older men with nutritional risk[32/49(65.3%)],it hasn't statistically difference(P=0.760).There was statistical differences in age(P=0.013),BMI(P=0.000),albumin(P=0.004)and hemoglobin(P=0.035)between women and men with nutritional risk.2.Compared with the without nutrition risk group,the scores of the comprehensive evaluation of the high nutrition risk group in the elderly were more serious,and the statistically difference was obviously in daily living ability score(P=0.020),and there was no significant differences in the scores of cognitive function and depression evaluation scores.3.The incidence of patients with nutritional risk group has coexisted in multiple diseases(x 2=12.551,p=0.000),multiplex medication(x 2=27.574,p=0.000),dental function(x 2=5.282,p=0.022),Visually impaired(x 2=5.046,p=0.025),chronic constipation(x 2=5.914,p=0.015),and sleep disturbance(x 2=3.895,p=0.048),which accounted for a large proportion,and there was statistically significant difference than patients without nutritional risk group.In this study,hearing impairment(x 2=0.119,p=0.73),urinary retention(x 2=2.164,p=0.144),chronic pain(x 2=2.656,p=0.103),cognitive function score(p= 0.310),depression score(p=0.970)and other aspects of nutrition risk group was not statistically significant.Multiple linear regression analysis showed that malnutrition of hospitalized patients in community was related to age,BMI,albumin,multi-disease coexistence,multiple drug use,and daily living ability.Conclusion:1.The incidence of nutritional risk or malnutrition among elderly inpatients in the community was 33%;2.The elderly in community hospitals have high nutritional risk or adverse morbidity,and they should conduct nutritional screening or assessment for geriatric patients who are attend community clinics;3.Age.BMI,albumin,multi-disease coexistence,multi-drug use,and daily living ability were the main influencing factors of nutritional risk or malnutrition in elderly inpatients in the community;4.A comprehensive elderly assessment of nutritional risk or poor patients can identify potential elderly problems in time.Early intervention for the elderly patients may improve their quality of life and prognosis.
