Objectives:To investigate the status quo of demands among the elderly in the institutions with integrated medical and nursing care,and explore the associated factors,to provide the basis for improving the happiness of the elderly and the service quality of institutions.Methods:Using cluster sampling,we conducted a questionnaire survey with abbreviated mental test,questionnaire on the demands and satisfaction of the elderly in urban integrated medical and nursing institutions among 300 senior citizens from July 2017 to September 2018.The data were analysis by SPSS21.0 for descriptive statistics,single factor and correlation and multiple linear regression analysis of data.Results:1.There are 300 senior citizens were surveyed,female 170(56.7%),male 130(43.3%),average age(81.07±7.62),213(71%)of the elderly sufered from chronic diseases and needed to take medicine throughout the year.The top four chronic diseases for the elderly were hypertension,stroke,heart disease and diabetes.The main reason for the elderly to choose institutional endowment was that their children are busy with their work 189(63%),and personal willingness 109(36.3%).The top three influencing factors for the elderly to choose nursing homes were 238(79.3%)with medical and health conditions,102(34%)with environment,the attitude of medical staff with 86(28.7%).2.The demand level of the elderly is relatively low,with a total score of(66.99±6.33),the standardized score for overall demand was 53.6%.The elderly had higher scores in safety and Love and belonging needs.Safety needs(16.42±2.09),related to gender,age,marriage,children,visiting frequency,living and health(P<0.05).Love and belonging needs(14.99±2.44),related to gender,marital status,educational level and children visit frequency(P<0.05).Physiological needs(13.28±2.49),related to the gender,age,marital status,economic source and health status of the elderly(P<0.05).Self-esteem(1 1.20±3.51),related to cultural degree,occupation before retirement,income and living situation.self-actualization needs(11.10±2.63),related to gender,marriage,culture degree,occupation before retirement,income and living situation(P<0.05).Linear regression results showed that elderly' gender,educational level,age,number of offspring,living condition and health status,had influence on the demand(P<0,05).Conclusions:1.The elderly age generally on the high side,and with a variety of chronic diseases.Especially in elderly women,due to a longer life,high morbidity,and poor health,they need more spiritual comfort,financial support and daily life care.2.The demands of the elderly in the combined medical and nursing institutions can be basically met,the elderly had higher scores in safety and love and belonging needs.They had lower scores in physiological needs,self-esteem and self-actualization needs.3.The elderly satisfied with the service attitude of medical staff,institutional environment and medical services,they were less satisfied with the catering conditions,spiritual comfort services and entertainment facilities.4.The elderly as the growth of the age of self-care ability drops,increasing their physiological needs.The elderly with poor health condition had more demands of medical service needs and security needs.The elderly who were single,low degree of cultural and male,had higher spiritual needs.
