Objective:To compare the epitympanum diameter,aditus of antrum and the degree of mastoid protess gasification in normal adults and otitis media(OM)patients on the healthy side and the otitis media side using Cone Beam Computed Tomography(CBCT).This study provides a theoretical basis for a proper understanding of the prevention and treatment of OM.Methods:A retrospective analysis of 246 adults(aged 18-73)who attended the Otolaryngology Head and Neck Surgery Clinic from October 2017 to April 2019,and their temporal bone CBCT images were used as the research object.?According to the diagnostic criteria of otitis media and the temporal bone CBCT,there were no inflammatory lesions in the middle ear divided into the otitis media group and the normal group.The normal group had no inflammatory lesions in both middle ears.Among them,there were 132 cases in the otitis media group;114 in the normal group.To compared the epitympanum width,height and depth measured,and the aditus of antrum width between normal adults in age,gender,and side,and statistical analysis was performed.?According to whether unilateral ears have otitis media,132 cases of otitis media group(45 cases of bilateral otitis media,87 cases of unilateral otitis media)were divided into normal ear group and otitis media group,including 87 ears in normal ear group and 177 ears in otitis media group.The epitympanum height,depth and width,and the aditus of antrum width were measured respectively,and the degree of mastoid protess gasification was determined.The measurement results of the two groups were analyzed statistically.Results:?In normal adults,the values of the epitympanum diameter,the aditus of antrum width are relatively constant.There is no statistically significant difference between the two in terms of gender and side(P>0.05).And In adult patients of otitis media,there was also no significant difference in the aditus of antrum width between the healthy side and the otitis media side(P>0.05).?In adult patients of otitis media,the mean height,depth and width of the epitympanum were measured at 6.121±0.676mm,6.875±0.489mm,5.871 ±0.684mm on the healthy side;and 5.564±0.627mm,6.318±0.540mm,5.381±0.668mm on the otitis media side.There is a statistically significant difference in the data of the epitympanum diameter between the healthy side and the otitis media side(P<0.05).?In adult patients of otitis media,the mean height,depth and width of the epitympanum were measured at 5.963±0.707mm,6.613±0.612mm,5.673±0.692mm on the mastoid protess gasification well side;and 5.480±0.577mm,6.364±0.529mm,5.380±0.703mm on the mastoid protess gasification bad side;there is a statistically significant difference in the data of the epitympanum diameter between the mastoid protess gasification well side and mastoid protess gasification bad side(P<0.05).There was no statistically significant difference of the antrum width between the mastoid protess gasification well side and mastoid protess gasification bad side(P>0.05).Conclusion:1.In normal adults,the mean height,depth and width of the epitympanum and aditus of antrum width of had no lateral or gender differences.2.In adult patients of otitis media,the mean height,depth and width of the epitympanum in the healthy side were larger than the otitis media side,but there was no significant difference in the aditus of antrum width between the healthy side and the otitis media side.3.In adult patients of otitis media,the mean height,depth and width of the epitympanum in the mastoid protess gasification well side were larger than the mastoid protess gasification bad side,but the aditus of antrum width is not affected by the degree of mastoid protess gasification.4.The reason of upper epitympanum is susceptible to otitis media that may be the upper epitympanum causes the dysfunction of the tympanic ventilator,which further leads to the formation of negative pressure in the epitympanum,thereby developing inflammatory lesions in the middle ear.
