Clinical Study On The Treatment Of Female Macromastia With Lejour’s Mastoplasty Technique With Medial Pedicle And Vertical Scar

Abstract/Summary:
Objective:To compare improvement in symptoms,breast shape,incision scars,nipple-areola sensation and lactation function,investigate the clinical effect of medial pedicle and vertical scar in Lejour’s mastoplasty technique and judge whether this method is worth popularizing by following up 32 female patients with macromastia who received vertical bipedicle technique(arms-plumbing technique vertical double pedicle)or Lejour’s mastoplasty technique with medial pedicle and vertical scar.Method:Eliminate interfere of objective factors such as patients’ age,fertility condition,extent of breast hypertrophy,concomitant symptoms,operators,postoperative management.32 patients with macromastia were randomly divided into two groups.16 received medial pedicle and vertical scar method(observation group)and the other 16 received vertical bipedicle technique(control group).Postoperative complications,scars,breast shape,nipple-areola sensation,lactation function and improvement in symptoms etc.were evaluated by Vancouver scar scale,breast shape scale,breast objective data determination tables,postoperative nipple-areola sensation scale.In order to evaluate and compare the clinical effect of these two methods the data were evaluated for statistical significance by SPSS 18.0 software,and P<0.05 was considered statistically significant.Result:These two methods can significantly improve the symptoms and there was no significant difference.The average value of scar length of observation group control group was 30.0±2.0cm and 59.0±3.5cm respectively;the average of Vancouver scar scale was 3.5±0.5 and 4.3±0.3 respectively;the average value of breast shape scale was 96.43±0.5 and 87.25±0.8 respectively;the rate of lactation function impairment was 0% and 2.77% respectively;the complication rate was 0%and 9.38 respectively.From these test data analysis and comparison,Lejour’s mastoplasty technique with medial pedicle and vertical scar makes more beautiful breast shape,less postoperative complications,shorter incision scars easily concealed,and it can also preserve nipple-areola sensation and lactation function;patients are more satisfied with this operation.Conclusion:Lejour’s mastoplasty technique with medial pedicle and vertical scar is safer and more reliable with better effect,especially suitable for patients with hypermastia in medial to heavy degree and mastoptosis.Compared with vertical bipedicle technique,the effect of Lejour’s mastoplasty technique with medial pedicle and vertical scar is better,worth spreading in clinical practice.The reasons are as follows: simpler preoperative design,lower difficulty in operation;less postoperative incision complications,smaller scars,incision location concealed easily;flexible breast,stable shape;the design of nipple-areola dermal pedicle is more suitable for breasts according to anatomical characteristics of breasts,which can provide rich blood supply,reduce damage to vital nerves and mammary ducts and guarantee safety of the operation;symptoms and quality of life are significantly improved.
