The Early Intervention Effect Of Zaoshiyulou Decoction On Postoperative Wounds With Wet-toxin-associated High Anal Fistula:A Clincal Observational Study

Objective:By observing the clinical effect of early intervention of Zaoshiyulou Decoction on postoperative wounds with wet-toxin-associated high anal fistula,further explore the clinical experience of the instructor using traditional Chinese medicine to manage wounds after anal fistula operation.Methods:A total of 60 patients with high anal fistula treated in the Department of Anorectal Surgery in Nanjing Traditional Chinese Medicine Hospital of the National TCM Anorectal Medical Center from February 2019 to November 2019 were selected and divided into two groups according to the random number table method,with 30 patients in each group.All patients were treated with anal fistula incision and hanging the control group was treated with antibiotics and conventional in-hospital preparations for fumigation,and the test group additionally took Zaoshiyulou Decoction on this basis.Record the changes in the total score of the internal dampness-toxicity syndrome before and after the operation in both groups.On the 2nd,7th,14th,and 21st days after surgery,observe and record the longitudinal diameter reduction rate,wound surface area reduction rate,wound secretion,granulation color,granulation density,and postoperative pain scores.Record the incidence of adverse events,evaluate the overall efficacy on the 21st day after surgery,and record the time to recovery.Results:The comparison of the gender,age,initial wound diameter and surface area of the two groups before surgery showed no statistical difference(P>0.05).There was no statistical difference in the total score of the internal dampness-toxicity syndrome between the test group and the control group before and 2 days after the operation(P>0.05),while the difference between the two groups on the 7th,14th and 21st days after surgery was statistically significant(P<0.05).On the 7th day after surgery,there was no significant difference in the longitudinal diameter reduction rate and surface area reduction rate of the two groups(P>0.05),and the difference was statistically significant on the 14th and 21st days after surgery(P<0.05).There was no significant difference in wound secretion,wound granulation density,wound granulation color,and postoperative pain on the 2nd and 7th day after surgery(P>0.05),the difference was statistically significant on the 14th and 21st days after surgery(P<0.05).After 3 weeks,the efficacy of the two groups was comprehensively evaluated.The effective rate in the experimental group was 97%and the control group was 87%.The adverse event rate in the experimental group was 10%and the control group was 6.7%.There was no statistical difference between the two groups in statistical analysis(P>0.05),and no adverse event seriously affecting the progress occurred.The average time to cure in the final test group was 87.7±3.9 days,and the control group was 91.4±8.5 days,with statistical differences(P<0.05).Conclusion:Zaoshiyulou Decoction significantly improves the clinical symptoms of patients with wet-toxin-associated high anal fistula after surgery,promotes the normal growth of new granulation tissue,reduces pain,shortens the healing cycle,and is safe and effective.
