Objective:To study the correlation between D-dimer and preeclampsia(PE),especially the difference between early PE and late PE,and analyze the change of D-dimer level in coagulation and fibrinolysis,so as to provide a test basis for better intervention and treatment of PE.Method :According to the inclusion criteria and exclusion criteria,78 pregnant women with PE diagnosis were randomly selected as the experimental group,which was divided into experimental group 1(early PE group,gestational age ? 20 weeks,< 34 weeks)and experimental group 2(late PE group,gestational age? 34 weeks).At the same time,80 healthy pregnant women in our hospital were randomly selected as the control group.The control group was also divided into control group 1(gestational week ? 20 weeks,< 34 weeks)and control group 2(gestational week ? 34 weeks).Height,weight,blood pressure,D-dimer(D-D),coagulation index and platelet related parameters were measured.The coagulation indexes were measured with Beckman Kurt ACL 9000 automatic blood coagulator: prothrombin time(PT),international standard ratio(INR),activated partial prothrombin time(APTT),plasma fibrinogen(FIB),thrombin time(TT).ACL TOP 3000 CTS automatic coagulation analysis and D-D of werfen company in the United States,and ACL 9000 automatic coagulation instrument in Beckman Kurt company in the United States to detect platelet related parameters:platelet count(PLT),mean platelet volume(MPV),platelet distribution width(PDW).Then analyze the differences between the data of each group to verify the correlation between D-dimer and preeclampsia.Result :The body mass index(BMI),D-D,SBP,DBP and MPV of experimental group 1 were higher than those of control group 1(P< 0.05).BMI,SBP and DBP in experimental group 2 were higher than those in control group 2(P < 0.05).D-D of experimental group 1 was higher than that of experimental group 2(P < 0.05).BMI,D-D,DBP and MPV of control group 2 were higher than those of control group 1(P< 0.05).BMI,D-D,SBP,DBP and MPV in the experimental group were higher than those in the control group(P< 0.05).There were significant differences in PT,INR,APTT,FIB,TT between the experimental group and the control group(P< 0.05).There were significant differences in PT,INR,FIB,TT between the experimental group 2 and the control group2(P< 0.05).There was significant difference between the experimental group 1 TT and the experimental group 2(P< 0.05).There was significant difference in FIB and TT between control group 1 and control group 2(P< 0.05).There were significant differences in PT,INR,APTT,FIB and TT between the experimental group and the control group(P< 0.05).D-D was negatively correlated with PT,INR,APTT,TT and PLT(r=-0.235,-0.247,-0.236,-0.351,-0.286,all P < 0.05).There was no correlation between D-D and FIB,MPV,PDW in PE patients(r =-0.129,0.084,0.126,all P>0.05).Conclusion:1.The D-D value of PE patients is significantly higher than that of healthy pregnant women.The hypercoagulable state of PE patients is serious,which is easy to cause thrombosis and thrombotic diseases.2.The D-D value of early-onset PE patients is higher than that of late-onset PE patients,which indicates that the earlier the onset of PE,the more serious the hypercoagulable state,and the more likely to cause adverse pregnancy outcomes of mothers and infants.It is necessary to strengthen the monitoring of early-onset PE.3.There is correlation between D-D and coagulation index and platelet related parameters in PE patients.D-D can be used as a laboratory index to detect coagulation function,which plays an important role in reflecting the hypercoagulable state and diagnosis and treatment of PE...
