Object:To investigate the relationship between serum alkaline phosphatase(ALP)level and occurrence,infarct area and prognosis of patients with acute cerebral infarction.Method:From October 2018 to May 2019,70 patients with acute cerebral infarction(ACI)in the department of general medicine,neurology department and emergency medicine of the first affiliated hospital of bengbu medical college were selected as the acute cerebral infarction(ACI)group,choose healthy check-up 50 cases as control group during this period,the record of two groups of patients with general information(including gender,age,smoking history,drinking history),medical history(high blood pressure,diabetes,heart disease,etc.),two groups are fasting 12 h after extraction of venous blood on an empty stomach take serum to check 3 ml centrifuge,Serum ALP,High Density lipoprotein-cholesterol hdl-c,TriglyceridesTC,Alanine transaminase ALT,Glutamic Oxaloacetic TransaminaseAST and c-reactive were determined by automatic biochemical analyzer and corresponding reagent in our hospitalProteinCRP,Total Choleswterol(TG),Low Densuty Lipoprotein(ldl-c)levels.According to the results of CT and MRI,the case group was divided into large area cerebral infarction[13](>3.0cm and the blood supply area of the major vascular trunk of more than 2anatomical sites),medium area cerebral infarction(1.5-3cm)and lacunar infarction(<1.5cm)by Adama classification method..By the National institutes of Health stroke scale(National Institute of Health stroke scale,NIHSS)score difference and pap Index(Barthel Index,BI)in patients with cases detailed physical examination and medical history of questioning,condition assessment,all according to the treatment of cerebral infarction patients after 14 d,NIHSS score difference and BI Index score,divided into good prognosis and bad prognosis.Result:1.There were no significant differences in gender,age,diabetes,heart disease,smoking history,drinking history,hdl-c,TC,ALT and AST between ACI group and control group.The serum levels of ALP,CRP,TG and ldl-c in patients with hypertension in the ACI group were higher than those in the NACI group.2.The difference between non-large-area group and large-area group was statistically significant(P<0.05);The difference between the non-large-area group and NACI group was statistically significant(P<0.05).3.The serum ALP level of the patients with good prognosis was(80.61±21.78)U/L,while that of the patients with poor prognosis was(93.75±18.58)U/L,the latter was significantly higher than the former,and the difference was statistically significant(P<0.05).4.Pearson correlation analysis was conducted on the relationship between ALP and cerebral infarction area and prognosis of the patients,and it was concluded that ALP was correlated with infarction area and prognosis of the patients,with statistically significant results(r=-0.278,r=0,292,P < 0.05).5.Logistic regression analysis suggested that ALP,hsCRP and ldl-c were risk factors for acute cerebral infarction(P<0.05),with statistical significance.6.ROC curve analysis of ALP,hs-crp and ldl-c,the influencing factors of cerebral infarction,showed that when A and ALP were 71.5U/L,the youten index reached the maximum(0.249),the sensitivity was 71.9%,and the specificity was 52%.This is useful for diagnosing cerebral infarction.Conclusion : 1.Serum ALP level is related to the occurrence of acute cerebral infarction,a risk factor for patients with acute cerebral infarction,and is related to the severity of cerebral infarction2.Serum ALP level was positively correlated with the infarct area in patients with acute cerebral infarction.The larger the infarct area,the higher the serum ALP level.3.It can be considered that the serum ALP level of patients is positively correlated with the prognosis of patients.The higher the serum ALP level,the higher the level of neurological impairment and the lower the self-care ability of patients,indicating that the prognosis is also worse.
