Objective:Acute cerebral infarction?ACI?is a sudden cerebral blood circulation disorder caused by arterial stenosis or occlusion due to various reasons.It is characterized by transient or permanent neurological dysfunction,with an extremely high mortality and disability rate,which imposes a great economic burden and mental stress on the society and their families.Therefore,the research hotspots in recent years focus on the research and development of ACI risk factors,as well as the improved function for ACI early risk assessment,severity assessment and prognostic biomarker role.Methods:All experimental specimens were collected from 108 ACI patients?case group?who were treated in the ward of Neurology Department,Central Hospital of Shenyang Medical College from August 2016 to October 2017,and 30 healthy subjects during the same period?control group?.In the case group,5 ml of venous blood was collected on the day of admission,and 5 ml of fasting elbow venous blood was collected in the morning of the 7th and 14th days after admission.For the control group,5 ml of fasting venous blood was collected on the day of physical examination.All blood samples were placed and then centrifuged,after the supernatant was removed,serum OPG level was measured using ELISA method.After the assay data were obtained,a statistical analysis was performed to compare serum osteoprotegerin?OPG?levels between ACI case group and healthy control group.Furthermore,the correlation of serum OPG level with the severity of neurological impairment,location and size of ACI,as well as prognosis was analyzed.Results:?1?Serum OPG levels of ACI cases on the 1st,7th and 14th days after admission were 553.44±34.49 pg/ml,478.67±27.36 pg/ml,431.18±24.36 pg/mL,which was higher than that of healthy controls,65.71±7.77 pg/ml.Linear analysis results demonstrated a statistically significant difference between case group and control group.?2?Serum OPG levels were statistically different among ACI cases with different OCSP types.The TACI group?565.55±36.36 pg/ml?was higher than other groups?PACI,551.55±30.39 pg/ml;POCI,551.08±27.95 pg/ml;LACI,532.57±30.28 pg/ml?,with statistical significances.?3?The duration of OPG high expression was associated with the infarct size:the infarct size was positively correlated with serum OPG level;the larger ACI area was,the longer serum OPG high expression persisted.?4?On the 1st,7th,and 14th days after admission,NIHSS scores of ACI cases showed that,serum OPG levels decreased along with the decline of NIHSS score,indicating that serum OPG levels were positively correlated with the severity of neurological impairment.?5?During the follow-ups,ACI cases were classified into good prognosis group and poor prognosis group,by using the BI grouping method.Serum OPG level was 566.08±32.82 pg/ml in the poor prognosis group,which was higher than good prognosis group?541.69±31.99 pg/ml?,and the difference was statistically significant.Conclusion:Serum OPG level is closely related to the occurrence,severity,infarct size and prognosis of ACI.It can be used as a reference indicator for predicting ACI severity and assessing its prognosis.
