Association Between Serum Alkaline Phosphatase And Cognitive Impairment In Ischemic Cerebral Small Vessel Disease

Objective:To investigate the correlation between Cerebral Small Vessel Disease and its cognitive impairment and serum alkaline phosphatase(ALP),to discuss their relationship,and early intervention measures were given to identify the risk factors of CSVD and its cognitive impairment in the early stage.Methods:A total of 212 inpatients with ischemic cerebrovascular disease admitted to the department of neurology of the first hospital of jilin university were collected.According to the Montreal cognitive assessment scale(Mo CA),the subjects were divided into the group with normal cognitive function of cerebrovascular disease(CSVD)and the group with cognitive dysfunction of cerebrovascular disease(CSVCI).According to the head MRI findings,the patients were divided into LI group and LA group,and LI and LA were graded.General information of the patients included: gender,age,smoking history,drinking history,hypertension,diabetes and history of coronary heart disease.Biochemical tests included cholesterol,LDL cholesterol,glycosylated hemoglobin,hypersensitive c-reactive protein,homocysteine,Ca2+,and ALP.The correlation between the ALP level and the relevant influencing factors was compared between the CSVD group and the CSVCI group,and the correlation between the severity of LI and LA and the ALP level was analyzed statistically.Results:1.Compared CSVD group with CSVCI group,age and hypertension were statistically significant(P < 0.05),sex ratio,coronary heart disease,smoking and drinking was not statistically significant(P > 0.05).Serum ALP of CSVCI group and CSVD group were significantly different(P < 0.05).There was no significant difference in LDL-C,total cholesterol,homocysteine,calcium ion and hypersensitive c-reactive protein between the two groups(P > 0.05).2.The enrolled patients were divided into group A,group B and group C according to ALP < 67u/L,67u/L < ALP < 84u/L and ALP > 84u/L,with 70 patients in each group.The incidence of LI and WMH in the three groups was compared.The results showed that three groups were significantly different in the incidence of LI and WMH(P < 0.05).3.Data from groups A,B and C will be compared pairwise to infer whether there is A difference in the incidence of LI and WMH between the two concentrations.The results showed that in the incidence of LI,there was A difference between group A and group B or group C(P < 0.05),group B and group C was not statistically significant(P > 0.05).group C and group A were statistically significant in the incidence of WMH(P < 0.05),group B and group A or C was not statistically significant(P > 0.05).4.Logistic regression analysis between cognitive impairment and its influencing factors in patients with cerebral small vascular disease showed that age and ALP were independent influencing factors of cognitive impairment in CSVD patients(P < 0.05).5.WMH severity and serum ALP were positively correlated with cognitive impairment in cerebrovascular diseases(r WMH=0.446,P=0.000,r ALP=0.663,P = 0.000).Conclusion:1.Age,hypertension and ALP are correlated with cognitive impairment ofCSVD,and age and ALP are independent risk factors for cognitiveimpairment of CSVD.2.High levels of ALP may be involved in the development of LI and WMH cognitive impairment in small cerebral vascular disease.
