Construction And Effect Evaluation Of Bundles Of Care Strategy Of Stage ? Cardiac Rehabilitation For Patients After Cardiac Valve Replacement

Objectives:In this study,bundles of care strategy of stage I cardiac rehabilitation for patients after cardiac valve replacement was constructed and applied to clinical practice.To exp-lore the effects of bundles of care strategy in stage I cardiac rehabilitation of patients after cardiac valve replacement.To provide theoretical and practical basis for clinical nurses,nursing administrators and patients to carry out stage I cardiac rehabilitation.Methods:Using the PIPOST model of JBI evidence-based health care center to construct evidence-based problems.The related studies on stage I cardiac rehabilitation of patie-nts after cardiac valve replacement were systematically searched in domestic and forei-gn databases,and the literature that met the inclusion criteria was screened out.Indepe-ndent quality evaluation,evidence extraction and evidence synthesis are carried out by researchers with evidence-based evaluation qualifications by using corresponding eval--uation tools for the included literature.Finally,bundles of care strategy for stage I car-diac rehabilitation after cardiac valve replacement was developed.The design of non-phase control study was adopted.66 patients with cardiac valve replacement in cardio-thoracic surgery department of a Grade 3 A Hospital in Jingzhou City from January 2018 to December 2018 were selected as the control group for historical review.68 patien-ts underwent cardiac valve replacement as the experimental group from January 2019 t-o December 2019.The control group adopts routine nursing measures after cardiac va lve replacement,the experimental group was treated with bundles of care measures,inc-luding evaluation,comprehensive respiratory function training,exercise,exercise obs-ervation and health education.Comparing before intervention and 2 weeks after interve-ntion cardiopulmonary function(heart rate,respiratory rate,blood pressure,PaO2,Pa CO2,SpO2,6MWD distance)and quality of life,comparing after intervention ICU stay time,ECG monitoring time,tracheal intubation time and drainage tube indwelling time in two groups of patients.Results:1.On evidence-based approach,this study constructed the bundles of care strateg-y for cardiac rehabilitation on stage I after cardiac valve replacement based on assess-ment,comprehensive respiratory function training,exercise observation and healh ed-ucation as the main body.2.Comparison of cardiopulmonary function between the experimental group and control group:? Before intervention,the cardiopulmonary function(heart rate,respira-tory rate,blood pressure,PaO2,PaCO2,SpO2,6MWD distance)of the two groups wer-e not statistically significant(P>0.05).?After intervention,there were significant dif-ferences in the cardiopulmonary function(heart rate,respiratory rate,blood pressure,PaO2,PaCO2,SpO2,6MWD distance)of the two groups(P<0.05).3.Comparison of quality of life between the experimental group and the control group:?Before intervention,the dimensions and total scores of quality of life betwee-n the two groups were not statistically significant(P>0.05).?After intervention,the-re were significant differences in the dimensions and total scores of quality of life betw-een the two groups(P<0.05).4.After intervention,there were significant differences in ICU stay time,ECG m-nitoring time,tracheal intubation time and drainage tube indwelling time between the two groups(P<0.05).Conclusion:1.On evidence-based approach,this study constructed the bundles of carestrategy for cardiac rehabilitation on stage I after cardiac valve replacement based on assessme-nt,comprehensive respiratory function training,exercise observation and health educat-ion as the main body.2.The research in this paper shows that the bundles of care strategy of stage I car-diac rehabilitation after cardiac valve replacement can improve the cardiopulmonary fun ction,shorten ICU stay time,ECG monitoring time,tracheal intubation time and drainage tube indwelling time,improve the quality of life of patients and have a positiv-e guiding effect on clinical nursing.
