Objective:To investigate the correlation between the level of TSH and the severity of coronary artery disease in patients with coronary heart disease and subclinical hypothyroidism,and to explore the feasibility of TSH as an index to evaluate the severity of coronary artery disease,so as to provide more basis for clinical judgment of the severity of coronary heart disease.Methods:According to the inclusion criteria and exclusion criteria,patients with coronary heart disease were selected from the cardiovascular department of Shaoyang Central Hospital from August 1,2017 to August 1,2019.According to the results of thyroid function test,patients were divided into three groups: normal thyroid function group,mild subclinical hypothyroidism group and severe subclinical hypothyroidism group.The proportion of single,double and three coronary lesions in three groups were calculated,and Gen was performed The influence factors of gensini score were analyzed.Results:(1)There was no significant difference in age,body mass index,hypertension history,diabetes history,smoking history among the three groups(2)The proportion of women with subclinical hypothyroidism was higher than that of men(P<0.05),and the more severe subclinical hypothyroidism was,the higher the proportion of women was(P<0.05).(3)There were significant differences in triglyceride,total cholesterol and LDL among the three groups(P<0.05).The more severe subclinical hypothyroidism was,the higher triglyceride,total cholesterol and LDL were(P<0.05),but HDL was not related to the degree of subclinical hypothyroidism Meaning(P>0.05).(4)The incidence of three coronary lesions in normal thyroid function group,mild subclinical hypothyroidism group and severe subclinical hypothyroidism group were 30.51%,50.00% and 58.33%,respectively,with statistical significance(P<0.05);the incidence of multi coronary lesions in three groups was 65.96%,81.63% and 86.11%,respectively The difference was statistically significant(P<0.0125).(5)The gensini scores of coronary heart disease patients in normal thyroid function group,mild subclinical hypothyroidism group and severe subclinical hypothyroidism group were(32.36 ± 30.20),(41.82 ± 30.21),(52.88 ± 30.12)respectively,and there were significant differences among the three groups(P<0.05).(6)There was a positive correlation between genesni score and TSH level(r=0.294,P<0.05),and a correlation between genesni score and TSH level(OR:1.976,95%CI: 1.633-2.391,P<0.05).Conclusion:(1)Subclinical hypothyroidism may be one of the risk factors for the aggravation of coronary heart disease.(2)The higher the level of TSH in patients with coronary heart disease,the more serious the degree of coronary artery stenosis and the larger the scope of lesions.
