Objective:To study the expression of GGT and Apo B/Apo AI levels in patients with premature coronary heart disease(PCHD),and To explore the correlation between GGT and Apo B/Apo AI and the degree of coronary artery lesions in PCHD.Methods:Patients with chest pain suspected of premature coronary heart disease who underwent coronary angiography(CAG)in the Cardiovascular Disease Center of The Affiliated Hospital of Yan’an University from July 2020 to July 2021 were selected.A total of 260 cases were selected according to the admission criteria,including 158 males,aged 47.91±5.899 years;102 female patients were 57.56±5.543 years old.General clinical information and biochemical files of patients were gathered and clinical records were composed.As per CAG results,all enlisted patients were partitioned into PCHD group(n=189)and control group(n=71).The PCHD group included 73 cases of acute myocardial infarction(AMI),56 cases of unstable angina pectoris(UAP)and stable angia Pectoris,SAP)of 60 patients.The differences in the clinical data between the PCHD group and the CON group and the three different clinical types of gamma-glutamyltransferase(GGT),apolipoprotein B/AI(Apolipoprotein B/AI,Differences in the distribution of Apo B/Apo AI)levels.Spearman relationship examination was utilized to dissect the connection between’s GGT,Apo B/Apo AI and GENSINI score.PCHD group was divided into low GENSINI score group(94 cases)and high GENSINI score group(95 cases)(GENSINI score<34 points,≥34 points),The predictive value of plasma GGT,Apo B/Apo AI levels and high GENSINI rating groups was analyzed by ROC curve,and Logistic regression analysis was conducted to analyze the independent influencing factors of high GENSINI score.Results:1.The levels of GGT and Apo B/Apo AI in the PCHD group were higher than those in the CON group(32(24.5,41)vs 23(19,29),P<0.001及0.68(0.52,0.82),P<0.001).2.The GGT level was compared between the three different clinical types of PCHD,and the difference was significant(P<0.001).Pairwise comparison showed that the GGT level in the SAP bunch was less than that in the AMI bunch and the UAP bunch,and the distinctions were measurably critical(P<0.05),but there was no significant difference between the AMI group and the UAP group(P>0.05).There was no huge differencein the pairwise correlation of Apo B/Apo AI among the three different clinical types(P>0.05).3.Comparison of GGT and Apo B/Apo AI levels in patients with different number of lesions showed:three-vessel lesions>two-vessel lesions>single-vessel lesions(P<0.001).4.GGT,Apo B/Apo AI levels were positively correlated with GENSINI score((rs=0.482,P<0.001;rs=0.406,P<0.001).5.The levels of GGT and Apo B/Apo AI in the high GENSINI score group were higher than those in the low GENSINI score group(37(28,44)vs 28(18,38),P<0.05;0.90(0.71,1.07)vs 0.77(0.60,0.90),P<0.05).6.ROC curve analysis of the prediction of GGT and Apo B/Apo AI on the high GENSINI score group,the area under the curve was(AUC)=0.720(95%CI:0.649~0.792,P<0.001),(AUC)=0.660(95%CI:0.583~0.738,P<0.001).When the GGT is taken as the cut-off point of 33.5 U/L,the maximum Youden index is 0.345,(sensitivity 63.2%,specificity 71.3%);when Apo B/Apo A1=0.865,the maximum Youden index is 0.270,(sensitivity 56.8%,specificity 70.2%).7.Multivariate Logistic regression analysis showed that high GGT and Apo B/Apo AI levels were independent influencing factors in the high GENSINI score group(OR=3.212,95%CI=1.633-6.317,P=0.001;OR=2.424,95%CI=1.217~4.829,P=0.012).Conclusion:1.The plasma GGT level has a certain relationship with the clinical classification of PCHD.The GGT level in the SAP group is less than the AMI group and the UAP group;the Apo B/Apo AI level has nothing to do with the clinical classification of PCHD.2.Plasma GGT,Apo B/Apo AI levels were significantly increased in PCHD group,Plasma GGT,Apo B/Apo AI levels were moderately positively correlated with GENSINI score,When GGT≥33.5U/L,Apo B/Apo AI≥0.865,it indicates a higher risk of ischemia.
