The Clinical Observation On The Treatment Of Cold Dampness Stasis Type Peripheral Neuropathy Caused By Taxanes Or Platinum With External Washing Of Wenyang Huashi Tongluo Formula Combined With MeCobalamin Tablets

Objective:To treat cold dampness stasis type peripheral neuropathy caused by taxanes or platinum chemotherapy drugs or both of them with external application of Wenyang Huashi Tongluo formula combined with MeCobalamin tablets,and to observe and evaluate its clinical effect.Method:Select 60 cases of peripheral neuropathy caused by taxanes or platinum chemotherapy drugs or both and in accordance with traditional Chinese medicine(here in after referred to as"TCM")syndrome differentiation of cold dampness stasis,and randomly divide them into treatment group and control group,with 30 cases in each.Patients of both groups are given to take Mecobalamin tablets,0.5mg each time,three times a day.In the treatment group,Wenyang Huashi Tongluo formula is added to wash the affected hands and feet,20 minutes each time,once a day.Both groups take 14 days as one course of treatment,day 1 to day 14 is the first course of treatment,day 15 to day 21 is the intermittent period,day 22 to day 35 is the second course of treatment,and the whole course of treatment takes 35 days.The treatment of Wenyang Huashi Tongluo formula is stopped in the intermittent period,while Mecobalamin tablets are taken in both groups throughout the whole course without intermission.After the two courses of treatment,observe and compare the differences of peripheral neurotoxicity,Total Neuropathy Score-clinical(TNSc),TCM syndrome,Activity of Daily Living(ADL),Karnofsky Performance Status(KPS)and other relevant indicators between the two groups,record the adverse reactions of the patients,and analyze the observation indexes before and after treatment statistically.Results:Of the 60 patients included in the study,57 cases have completed the treatment,including 28 in the treatment group and 29 in the control group.The completion rate is high.The peripheral neurotoxicity,the TCM syndrome scores and the ADL of the two groups are obviously superior to those before treatment(p<0.05).After treatment,the TNSc of the treatment group is significantly lower than that before treatment and the KPS of the treatment is significantly taller than that before(p<0.05),both the TNSc and the KPS of the control group is statistically insignificant(p>0.05).The treatment group with a total effective rate of 71.43%is significantly better than control group's total effective rate of 31.03%(p<0.05).After treatment,the TNSc,the TCM syndrome therapeutic effect and scores of the treatment group are obviously superior to those of the control group(p<0.05).In the treatment group,the therapeutic effect of the TCM syndrome,the scores of limb numbness,limb pain,chilly limbs and adverse flexion and extension after treatment are significantly lower than those before treatment(p<0.05).In the control group,only the scores of limb numbness after treatment is better than that before treatment(p<0.05).The scores of chilly limbs and adverse flexion and extension in the treatment group are obviously lower than those in the control group,and the difference is statistically significant(p<0.05).The improvement of limb numbness,limb pain in the two groups is statistically insignificant(p>0.05).The therapeutic effect of the two groups both have no correlation with different chemotherapy drugs or different peripheral neurotoxicity(p>0.05).Conclusion:External application of Wenyang Huashi Tongluo formula combined with MeCobalamin tablets has significant effect on cold dampness stasis type peripheral neuropathy caused by taxanes/platinum chemotherapy drugs,can effectively improve TCM syndrome,improve life quality to some extent,and the treatment is relatively safe.Therapeutic effect has no correlation with different chemotherapy drugs or different peripheral neurotoxicity.
