Clinical Observation Of Qingfei Tuomin Decoction In Treating Children With Allergic Rhinitis And Eustachian Tube Obstruction

Objective:Allergic rhinitis(AR)is a common disease in the ENT department.Children's immune system is not fully developed or they are born with an allergic constitution.The incidence of allergic rhinitis in children is still high,which seriously affects children's health,academic performance and physical and mental development.Clinically,it is common that adenoidal hypertrophy(AH)occurs simultaneously in AR patients.Obstruction of eustachian tube,which is due to the physiological even inflammatory AH,might lead to secretory otitis media,tinnitus and hearing loss especially in severe cases.At present,western medicine mainly focuses on adenoidectomy and oral drug treatament,but the surgical risks and drug side effects make the families of most children more resistant.The purpose of this project is to observe the clinical efficacy of Qingfei Tuomin Decoction in the treatment of allergic rhinitis in children with pulmonary meridian fever and eustachian tube obstruction,and to provide effective Chinese medicine treatment for such children.Research methods:From March 2019 to February 2020,60 cases of allergic rhinitis with eustachian tube obstruction in children of 3-6 years old with pulmonary meridian fever type are selected in the otolaryngology clinic of Jiangsu Provincial Hospital of Traditional Chinese Medicine.They are divided into two groups,30 patients in each group,through analysis of the difference between the two groups of patients,such as the severity of disease,age distribution,etc.Besides,there is no significant difference that which group is given Qingfei Tuomin Decoction orally;the control group is taken oralma drops combined with cetirizo hydrochloride orally or with mometasone furoate spray for external treatment.The course of treatment in both groups is 28 days,and the symptoms,physical scores,and acoustic impedance test results are recorded on the 14th and 28th days after drug administration to evaluate the efficiency.Results:1.After 4 weeks of treatment,there is a significant difference in the scores of the main symptoms and signs between the control group and the treatment group and the Unprocessed(P<0.01),indicating that both treatment schemes have a therapeutic effect on these diseases.2.After 2 weeks of treatment,there are significant differences in sleep snoring symptoms between the two groups(P<0.05),and the treatment group is better than the control group.After 4 weeks of treatment,there are significant differences in runny nose,nasal congestion symptoms among the two groups(P<0.05).The treatment group is better than the control group.3.After 2 weeks of treatment,the effective rate of the treatment group,the control group and P are 16.67%,6.67%,0.228(P>0.05)respectively.No significant difference between the two groups.Wait for 4 weeks to end the intervention,the effective rate of the treatment group,the control group and P are 86.67%,83.33%,0.765(P>0.05)respectively,there is no significant difference as well.4.After 4 weeks of treatment,the acoustic impedance test results showed that 18 cases are cured in the treatment group,2 cases are markedly effective,3 cases are effective,7 cases are ineffective,the total effective rate is 76.67%.While 14 cases are cured in the control group,4 cases are markedly effective,and 1 is effective.There are 11 cases ineffective,the total effective rate is 63.33%,P=0.383.As a result,there is no significant difference between two groups.But Qingfei Tuomin Decoction showed an advantage over the control group in improving the symptoms of eustachian tube obstruction in children.Conclusion:Qingfei Tuomin Decoction for children with allergic rhinitis combined with eustachian tube obstruction can improve children's main symptoms such as runny nose,nasal congestion,etc.At the same time,it can improve the symptoms of sleep snoring and mouth breathing in children,and reduce the severity of eustachian tube obstruction.Compared with the western medicine group,the traditional Chinese medicine has fewer side effects,avoiding the risks and complications after surgery.And it improves the appetite,immunity,and long-term efficacy of some children.It is worthy of widespread application in the clinic.Acoustic impedance examination has certain significance for indirectly reflecting the severity of eustachian tube obstruction and the necessity of adenoid surgery.Compared with other examinations,it has the advantages of simplicity,speed,sensitivity,economy,and no radiation.As a result,it is worth promoting.
