Clinical Observation And Methodology Discussion Of Tuomin Rhinitis Ointment In Preventing Seasonal Allergic Rhinitis In Children

Objective:1 To evaluate the preventive effect of Tuomin Biyan Cream in reducing the recurrence rate,reducing the degree of relapse and improving the quality of life of children with seasonal allergic rhinitis.2.To understand the effect of Tuomin Biyan Cream on intestinal flora of children with allergic rhinitis.To search the operational taxonomic units of intestinal microflora in children and to compare and analyze the differences of intestinal microflora structure in children before and after taking cream.3.To explore the practical problems in the process of taking Tuomin Biyan Cream,such as whether the children accept the taste of desensitized rhinitis cream,the influence of the formula on the overall health of the children,and parents' degree of acceptance.Methods:1.Children with allergic rhinitis were recruited from Beijing Huimin Children's Hospital of TCM and Beijing Yanhuang Hospital of TCM.Children began to take Tuomin Biyan Cream one month in advance.This cream prescription is based on the constitution characteristics of children with allergic rhinitis,combined with the specific clinical manifestations.Twice daily for 30 days.The previous symptoms and the current year symptoms were recorded before and after taking the cream.And the total effective rate was converted according to the score reduction rate.The PRQLQ scores of children with allergic rhinitis were recorded before and after the prescription.SPSS software was used for statistical analysis of the data.2.30 fresh feces samples were put on sterile tube and saved by researchers in-80?refrigerator.The composition of intestinal microbial community was analyzed by 16SrRNA sequencing.LEfSe method was used to explore the significance of OTU difference between the two groups of samples.After obtaining the overall Beta diversity index of the samples,PERM ANOVA method and ANOSIM method were used to compare the microbial composition of the feces samples of the children before and after taking cream.3.Semi-structured qualitative interviews were organized.In all children participating in the case observation study,continuous sampling was used to organize face-to-face or telephone interviews.The main content of the interview involves whether the children accept the taste of the cream,whether they could be guaranteed to take the cream on time,and what are the advantages of the cream compared with traditional Chinese medicine forms.Transcribe all the interview recordings into words,then form the theme and summarize.Results:1.Among the 45 patients,there were 10 significant cases,21 effective cases and 14 invalid cases.The overall effective rate was about 68.89%.The total score of PRQLQ questionnaire for children decreased from 58.09�20.00 to 38.20�16.35,and the difference was statistically significant(P<0.01).Tuomin Biyan Cream can improve the quality of life of children in nasal symptoms,eye symptoms,other symptoms,rhinitis related behavior and emotional response,with statistically significant differences in scores before and after(P<0.01).2.The number of specific OTU was 364 in the fecal samples of the children before the prescription,and 502 in the fecal samples of the children after the prescription.The number of OTU contained in both samples was 452.There were 15microorganisms with relatively high abundance in the fecal samples before taking cream,and 6 in the fecal samples after taking cream.Among them,Faecalibacterium,Weissella,Bifidobacterium,Enterobacter,Blautia,Lachnospira,Klebsiella and Coprococcus in the fecal samples of the children before,Acidaminococcus and Prevotella from fecal samples of the children after were specified to the genus level.Both PERMANOVA method and ANOSIM method were used to verify that there were statistical differences in the flora structure of fecal samples before and after(p<0.05).3.13interviewees participated in the interview,and the results showed that parents were generally lack of prevention awareness of AR,such as" This is the first time to prevent rhinitis for children."Children accept paste taste,as a parent said,"It is not difficult,my child can take and there is no resistance to taking medicine now." Parents tthought the most important advantage of Tuomin Biyan Cream is convenient,as a parent said,"It is very convenient,we do not need to boil traditional Chinese medicine."The overall health of the children has also been improved,including appetite and sleep,etc.As a parent said,"My ch ild can sleep until dawn,very stable".Conclusions:1.The prevention effect of Tuomin Biyan Cream is good.It can effectively reduce the recurrence rate of children with seasonal allergic rhinitis,reduce the degree of illness after recurrence,and comprehensively improve the quality of life of children in the allergy season,which is worthy to be popularized and referenced.2.Tuomin Biyan Cream can also improve the intestinal environment of children and correct the imbalance of bacterial flora,which is of certain value for further study.3.Tuomin Biyan Cream is highly accepted by children due to its good taste,and the medication compliance of children is guaranteed.Because it is convenient to store and take,it is also recognized by the parents of children.The overall health of the children has also been improved.
