ObjectiveThrough investigation and analysis of chronic obstructive pulmonary disease(COPD)of residents in Zhenzhou town,Yizheng,Yangzhou city and then comparing the results with those of residents in a rural area in Baoying district of Yangzhou city.The results can provide reliable reference for the prevention and treatment of COPD in Zhenzhou town and even the whole city of YangzhouMethodsThis study is a cross-sectional study in which residents aged 40 and over in 8 districts of Yizheng county are selected by stratified overall random sampling.Meanwhile,the subjects are investigated by means of the unified pulmonary function examination instrument and the unified questionnaire which is discussed and approved by the professional doctors of the Department.The residents in this area are stable,the medical resources meet the daily needs,and the investigation objects have a high degree of cooperation.The relevant functional departments understand and can cooperate to complete the relevant investigation,which basically meets the requirements of epidemiological investigation.1.The basic contents of the questionnaire include:the basic situation of patients,smoking status,household biofuel use,household ventilation status,outdoor activity status,dust exposure,personal basic disease and family disease history,daily physical work,etc.2.The diagnosis standard of COPD adopted in this study refers to the global initiative for COPD(gold)[1]and the guideline for diagnosis and treatment of COPD(2013 Revision)formulated by the Chinese Medical Association[2]:(1)air flow persistence:FEV1/FVC(forced expiratory volume/forced vital capacity in the first second)<70%after inhalation of bronchodilator,and it can be determined that there is continuous air flow restriction;(2)According to the patient's past medical history,symptoms and physical signs,the patient was diagnosed as COPD on the basis of comprehensive diagnosis excluding other diseases that may cause related symptoms.3.Collecting relevant data,establishing Excel database and using SPSS 19.0 software for data analysis.?2 test was used for single factor analysis.At the same time,logistic multi factor analysis was used to analyze the relationship between the risk factors and the prevalence of COPD in this area.?=0.05 was used as the test level,and adjusted OR value and P value for the risk assessment of related factors.4.At the same time,we compared the prevalence and risk factors of chronic obstructive pulmonary disease(COPD)with those of a rural area in Baoying district of Yangzhou city.Results:1.758 permanent residents aged 40 and above participated in the survey,and the final information collection was complete.At last,628 people met the quality control requirements for lung function test,with an average age of 49.6±4.5 years.There were 367 males(58.4%)and 261 females(41.6%).A total of 49 patients met the diagnostic criteria of COPD,with a total prevalence of 7.8%.2.Univariate analysis showed that factors associated with COPD were sex(?2=9.791,P<0.05)?age(?2=17.171,P<0.05)?Smoking history(?2=22.960,P<0.05)?Smoking index(?2=71.895,P<0.05)?Smoking years(?2=72.332,P<0.05)?Education(?2=8.116,P<0.05)?Kitchen fuel(?2=8.586,P<0.05)?Dust exposure history(?2=5.559,P<0.05)?Personal history of respiratory diseases(?2=25.153,P<0.05).3.Multivariate logistic regression analysis was used to analyze the variables with statistical significance in single factor analysis.The results showed that:age(OR 1.126,95%CI 1.037-1.223),smoking history(OR 9.692,95%CI 1.566-59.978),smoking index(OR 1.003,95%CI 1.000-1.006),smoking year limit(OR 1.096,95%CI 1.023-1.174),education background(OR 3.839,95%CI 1.623-9.082),Personal history of respiratory diseases(OR 0.080,95%CI 0.016-0.404)is the risk factor of COPD.4.The prevalence rate of COPD in the area of Zhenzhou town,Yizheng,Yangzhou city was 7.8%and the risk factors of COPD in this area included:age,smoking history,smoking years,smoking index,education background and personal history of respiratory diseases.The prevalence rate of COPD in a rural area in Baoying district of Yangzhou city was 17.6%and the risk factors included:male,smoking history,cooking history,family history of respiratory diseases.The prevalence and risk factors of COPD were different between the two regions.Conclusion:1.The results of this research showed that the prevalence of COPD was 7.8%in the population aged 40 and over,which was lower than the national average(13.7%).The prevalence of COPD in men was higher than that in women,and the prevalence of COPD increased with age.2.Advanced age,smoking,smoking years,smoking index,low education background and personal history of respiratory diseases are the risk factors of COPD in this area3.The prevalence and risk factors of COPD in different regions of Yangzhou are different.4.The pulmonary function of patients with COPD in rural areas of Yangzhou is worse than that in urban areas.
