| Objectives To investigate the prevalence of frailty in elderly patients with chronic obstructive pulmonary disease(COPD)and analyze the risk factors of frailty in elderly patients with COPD,in order to provide a new idea for the prevention of COPD patients frailty and chronic disease management.Methods According to the inclusion and exclusion criteria,a total of 247 COPD patients were selected.General conditions,laboratory,pulmonary function and imaging data of subjects were recorded.All patients were assessed for frailty according to the Fried standard,and the assessment results were recorded.Based on the assessment results,the selected subjects were divided into the frailty group(≥3 Fried criteria)and the non-frailty group(<3 Fried criteria,of which meeting 0 Fried criteria means no frailty,meeting 1-2 Fried criteria means pre-frailty).SPSS23.0 software was used for statistical analysis to compare the differences between the two groups in general and other data,and to analyze the risk factors of frailty in elderly COPD patients.Results 1.Among 247 elderly COPD patients,the prevalence of frailty and prefrailty was 21.46% and 34.41%,respectively.2.Comparison of general information,severity of disease and symptoms,laboratory and imaging data: There were significant differences in age,BMI,educational level,comorbidities,multiple medication,weekly activity,sedentary time,GOLD grade,mMRC,TP,Alb,ALT,BUN,TG,CRP and PAH between frailty group and non-frailty group(P<0.05).3.Multivariate logistic regression analysis: Age(OR=1.104),GOLD grade severe or above(OR=4.072),sedentary behavior(OR=4.072),mMRC grade increased(OR=2.394),comorbidity(OR=5.422),multiple drugs(OR=3.028),and high CRP expression(OR=3.689)were independent risk factors for frailty in elderly patients with COPD(P<0.05).4.Correlation analysis of COPD related indicators and frailty: Among the elderly COPD patients,age(r=0.262),comorbidity(r=0.223),multiple drugs(r=0.433),sedentary behavior(r=0.415),GOLD grade(r=0.341),MMRC grade(r=0.486),CRP(r=0.197),PAH(r=0.315)were positively correlated with frailty(P < 0.05),and BMI(r=-0.324),education level(r=-0.156),weekly activity(r=-0.307),TP(r=-0.147),Alb(r=-0.246),ALT(r=-0.197)and TG(r=-0.241)were negatively correlated with frailty(P < 0.05).Conclusions 1.Frailty is very common in elderly COPD patients,and the prevalence of frailty was 21.46%,and pre-frailty was 34.41%.2.Age,GOLD grade severe or above,sedentary behavior,mMRC grade increased,comorbidity,multiple drugs,and high CRP expression were independent risk factors for frailty in elderly patients with COPD.3.The occurrence of frailty in elderly COPD patients was closely related to age,BMI,comorbidities,multiple drugs,education level,weekly activity,sedentary behavior,GOLD grade,MMRC,TP,Alb,ALT,BUN,TG,CRP,PAH. |
