Background:Chronic tonsillitis(CTS)is a chronic inflammation of tonsil caused by the repeated attack of acute tonsillitis.In clinic,surgical treatment is often used for radical resection,but patients are ofrten troubled by the discomfort of throat swelling and pain after operation,which seriously affects the recovery process after operation.With the development of medical technology,tonsillectomy has made correspond:ing improvement and innovation.However,in clinical research,there are still problems such as postoperative pharyngeal pain,mucosal swelling and discomfort.Therefore,the postoperative treatment of chronic tonsillitis should be paid attention to,but there is no clear guidance or consensus in this regard.The existing western medicine treatment has the problems of inconvenient operation and obvious side effects.Traditional Chinese medicine is gradually used in the treatment of chronic tonsillar surgery with its advantages of effectiveness and portability,so as to promote postoperative recovery.Purpose:In this study,through the observation of the clinical effect of modified kujiu Decoction in the treatment of chronic tonsillitis after operation and the study of its influence on inflammatory factors,the effectiveness of its clinical use is comprehensively evaluated,so as to provide a new method for the treatment of traditional Chinese medicine after tonsillectomy,so as to achieve the ultimate goal of reducing the pain of patients and accelerating the recovery of patients after operation.Methods:60 patients with confirmed chronic tonsillitis who were hospitalized between December 2018 and October 2019 were selected from the Department of Otolaryngology of Nanjing Traditional Chinese and Western Medicine Hospital affiliated to Nanjing University of Chinese Medicine.The patients who met the criteria were randomly divided into the control group and Chinese medicine treatment.There were three groups:gargle modified bitter liquor decoction and western medicine treatment group(containing gargle compound chlorhexidine gargle),with 20 cases in each group.The three groups of patients were bilaterally tonsillectomy under the general anesthesia by the same skilled surgeon.The same low-temperature plasma radiofrequency ablation system and blade were used during the operation.All patients received conventional anti-infection,hemostasis,and fluid replacement surgery.treatment.The western medicine treatment group started to gargle compound chlorhexidine gargle from the first day after operation;the Chinese medicine treatment group started to gargle daily modified bitter liquor from the first day after operation;the control group did not use any gargle.The treatment cycle is 5 days.The mucosal swelling time and pseudo-membrane detachment time of the three groups of patients were recorded respectively.The clinical efficacy was evaluated by visually scoring the pharyngeal pain on the first and fifth postoperative days.On the first and fifth postoperative days,venous blood from patients was collected and processed to obtain serum,and the expression of IL-6 and TNF-?in serum samples was detected by enzyme-linked immunosorbent assay(ELISA)Result:(l)After 5 days of treatment,the VAS scores of sore throat in the three groups were:4.10±1.12 points in the control group,3.30±1.30 points in the western medicine treatment group,and 2.15±1.14 points in the Chinese medicine treatment group.Compared with the control group,the Western medicine treatment group had a significant difference(P<0.05).The Chinese medicine treatment group had a significant difference compared with the control group(P<0.05).The Chinese medicine treatment group had a significant difference compared with the Western medicine treatment group(P<0.05)(2)After 5 days of treatment,the mucosal swelling time(median M)of the three groups was 3 days in the control group,2 days in the western medicine treatment group,and 2 days in the Chinese medicine treatment group.Compared with the control group,the Western medicine treatment group had a significant difference(P<0.05),and the Chinese medicine treatment group had a significant difference compared with the control group(P<0.05).The Chinese medicine treatment group had no significant difference with the Western medicine treatment group(P>0.05)(3)After 5 days of treatment,the pseudomembranous detachment time(median M)of the three groups of patients was 10.50d in the control group,10d in the western medicine treatment group,and 8d in the traditional Chinese medicine treatment group.There was no significant difference between the western medicine treatment group and the control group(P>0.05);there was a significant difference between the traditional Chinese medicine treatment group and the control group(P<0.05);there was a significant difference between the traditional Chinese medicine treatment group and the western medicine treatment group(P<0.05)(4)The patients were treated for 5 days after operation.The IL-6 expression(median M)in the serum samples of the three groups of patients on the 5th day after operation were 44.50 pg/ml in the control group and 39.43 pg/ml in the western medicine treatment group.,Chinese medicine treatment group 38.29pg/ml.The expression levels(median M)of serum samples in the three groups of patients on the fifth day after operation were:control group 24.19 pg/ml,western medicine treatment group 20.27 pg/ml,and Chinese medicine treatment group 20.87 pg/ml.Compared with the control group,the Western medicine treatment group has a significant difference(P<0.05);the Chinese medicine treatment group has a significant difference compared with the control group(P<0.05);the Chinese medicine treatment group and the western medicine treatment group have no significant difference(P>0.05)).Conclusion:Gargle Modified Kujlu Decoction has a certain effect on the treatment of chronic tonsillitis after operation.It reduces the level of sore throat in patients,relieves the swelling of the mucosa in the operation area,Promotes the shedding of pseudomembranes,and down-regUlate the inflammatory factors IL-6 and TNF-?.There are advantages in promoting wound healing.Traditional Chinese medicine is effective in the treatment of chronic tonsillitis after operation,and has clinical promotion.
