ObjectiveTo explore the current status of fear of cancer recurrence,identify the influencing factors of fear of cancer recurrence,and to verify the relationship between fear of cancer recurrence,illness perception and coping style in patients with bladder cancer.To greater clarity the real experience and feelings of fear of cancer recurrence in patients with bladder cancer by qualitative researchMethodIn the first part,315 patients with bladder cancer who were treated in urology ward and cystoscopy room of The First Affiliated Hospital of Medical School of Zhejiang University from August 2017 to June 2018 were recruited by convenience sampling.General information questionnaire,Fear of progression questionnaire short form,Brief illness perception questionnaire and Medical coping modes questionnaire were used as research tools to collect data.Statistical description,single factor analysis,correlation analysis,multiple linear regression analysis,mediation effect analysis were conducted using SPSS19.0In the second part,12 patients with bladder cancer were recruited for semi-structured interviews by purposive sampling.Data were collated,coded and analyzed by Nvivo 11.0.Result(1)The total score of fear of cancer recurrence in patients with bladder cancer was(28.13±10.42).25.7%of the total number of patients with fear of cancer recurrence reaching clinical level(total score?34).The highest score was:I become anxious if I think the disease may progress.(2)Univariate analysis showed that the total score of fear of cancer recurrence in bladder cancer patients was significantly different in gender,education level,first treatment,age,occupational status,family income and recurrence frequency(P<0.05).(3)The correlation analysis showed that fear of cancer recurrence was positively correlated with surrender(r=0.832,P<0.01),illness perception(r=0.808,P<0.01),and negatively correlated with confrontation(r=-0.570,P<0.01),avoidance(r=-0.340,P<0.01).Illness perception was negatively correlated with surrender(r=0.881,P<0.01),fear of cancer recurrence(r=0.808,P<0.01)was positively correlated with confrontation(r=-0.664,P<0.01)and avoidance(r=-0.430,P<0.01).(4)Multivariate regression analysis showed that surrender,age,illness perception and gender entered the regression equation,explaining 78.4%of the total variance of fear of cancer recurrence.The contribution degree of variation from large to small is:surrender,illness perception,age,gender.(5)Mediating effect analysis shows that illness perception can directly affect fear of cancer recurrence,the direct effect is 0.376.Illness perception can also indirectly affect fear of cancer recurrence through surrender,the indirect effect is 0.462,accounting for 55.18%of the total effect.Surrender plays a part of mediating role between illness perception and fear of cancer recurrence.(6)Analyze the interview data,seven themes were extracted:trigger factors,fear of death,loneliness,uncertainty,excessive attention,compromise and surrender,effect on life.Conclusion(1)The level of fear of cancer recurrence in patients with bladder cancer in China is high,which should be paid attention by clinical nurses.(2)Surrender,illness perception,age and gender are independent influencing factors of fear of cancer recurrence in patients with bladder cancer,surrender has the greatest impact on fear of cancer recurrence.(3)Fear of cancer recurrence can be triggered by specific situations,experiencing fear of death,loneliness,uncertainty,excessive attention,and surrender,which can lead to physiological changes and socioeconomic problems.
