Objective1.To investigate the status quo of fear of cancer recurrence,coping style,self-efficacy and social support level of breast cancer patients undergoing breast reconstruction2.To analyze the influence of coping style,self-efficacy,social support on the fear of cancer recurrence in patients after breast reconstruction and the correlation between coping style self-efficacy social support and the fear of cancer recurrence3.The structural equation model was constructed to analyze the functional relationship between the leading variables (self-efficacy,social support),the mediating variables (coping style) and the outcome variables (fear of cancer recurrence)Methods1.Using the method of convenience sampling,257 patients who were admitted to the department of breast reconstruction in Tianjin hospital from June 2019 to January 2020 after breast reconstruction were selected as the research objects The general information questionnaire,Concern About Recurrence Scale(CARS),Medical Coping Modes Questionnaire(MCMQ),General Self-Efficacy Scale(GSES),Social Support Rating Scale(SSRS) were used to conduct a cross-sectional survey of subjects meeting the inclusion criteria2.Excel software was used for data double-core pairing and double-entry,SPSS26.0 software was used for data analysis,mean standard deviation was used for measurement data,and percentage of frequency was used for counting data.The effect of general data on the fear of cancer recurrence in patients after breast reconstruction was analyzed using the T test or one-way analysis of variance Pearson correlation analysis was used to analyze the correlation between coping style,self-efficacy,social support and fear of cancer recurrence.Multivariate stratified regression analysis was performed on the fear level of cancer recurrence in patients after breast reconstruction using the variables with statistical differences in univariate analysis and correlation analysis as independent variables3.AMOS24.0 statistical software was used to construct a structural equation model to explore the functional pathway between the level of social support,general self-efficacy and the level of fear of cancer recurrenceResults1.In this study,270 questionnaires were distributed and 263 questionnaires were recovered,with the recovery rate of 97.41%.There were 257 valid questionnaires,with an effective rate of 97.72%.The level of fear of cancer recurrence in patients after breast reconstruction was moderate (55.96±24.72).The quartile spacing and median of the three dimensions of medical coping style and avoidance yield were 46(43,49),19(17,22),17(16,19),9(7,10).The quartile spacing and median of self-efficacy was 25(22,29) points.Social support level score (38.65±7.17)2.The risk of fear of cancer recurrence in patients after breast reconstruction was statistically significant in terms of age,education,marital status,medical expense payment,breast reconstruction and family tumor history.The lower the age,the higher the education level,the divorce,the lower the per capita monthly income of the family,the choice of prosthesis breast reconstruction and the family history of tumor,the higher the level of fear of cancer recurrence3.The results of correlation analysis showed that there was a positive correlation between the total score of cancer recurrence fear and the yield coping style of patients after breast reconstruction (P<0.05).It was negatively correlated with the total score of general self-efficacy (P<0.05).There was a significant negative correlation with the total score of social support and the utilization degree of subjective support and objective support (P<0.05)4.The results of multiple linear regression analysis show that patients with poor social support utilization,divorce,high school/junior college education,poor general self-efficacy,age over or equal to 56 years old,and family history of cancer,have higher level of fear of cancer recurrence.(Adjusted R2=0.516,P<0.001)5.Structural equation models suggested that yield coping style was partially mediated between social support for self-efficacy and fear of cancer recurrence in breast cancer patients after breast reconstruction,?2=3.243,P=0.072,df=1,?2/df=3.243,RMSEA=0.094,GFI=0.995,the model fitted wellConclusion1.Breast cancer patients after breast reconstruction has a higher level of fear of cancer recurrence,and the level of fear of cancer recurrence is high in patients with breast reconstruction2.The level of fear of cancer recurrence in patients with breast cancer after breast reconstruction was closely related to their coping style general self-efficacy and social support3.Some demographic data (age,education,marital status,medical expenses,medical expenses) and disease-related data (menopause,family history of tumor,recurrence of tumor) are the influencing factors of recurrence fear of breast cancer patients after breast reconstruction.The lower the age,the higher the education level,the divorce,the lower the per capita monthly income of the family,the choice of prosthesis breast reconstruction and the family history of tumor,the higher the level of fear of cancer recurrence4.Coping style played an intermediary role between general self-efficacy social support and cancer recurrence fear level in patients after breast cancer reconstruction.Self-efficacy social support was a prepositive variable affecting cancer recurrence fear and had both direct and indirect effects on cancer recurrence fear.
