| Objective:Sudden sensorineural hearing loss(SSNHL),refers to the sudden hearing loss within 72 hours,and at least in the two adjacent the frequencies hearing loss is greater than or equal to 20 dBHL,It's a kind of sensorineural hearing loss with unknown cause.There are many etiological factor which cause SSNHL,clinical studies points out that cardio-cerebral vascular diseases,viral infections,autoimmune diseases,infectious diseases,cancer etc,are telated to SSNHL.And,cardio-cerebral vascular disease-induced local circulation is believed to be a significant incentive for SSNHL and,itis an important part in the process and prognosis of SSNHL.Now in chronic diseases such as diabetes,hyperlipemia's incidenceshave been increased than before,the researches of their relationship with other disease-related are more deeply.This subject is to recall the patients with SSNHL in our hospital,analysis its chronic diseases incentives and the discussion with the comprehensive treatment of SSNHL with the systemic chronic diseases(diabetes,and hyperlipemia),then compared and analysis withthe SSNHL patientswithout chronic diseases.The objective is to discuss with the treatment methods of SSNHL with chronic diseases and its meaning.Method:Since June 2012-June 2016,We reviewed 196 cases of SSNHL in the department of otorhinolaryngology of our hospital with complete data,taking different treatment options according to different groups.All patient were divided into 3 groups:diabetes group,hyperlipemia groupand control group,evaluate the age?gender?onset time,with or without diabetes or hyperlipemia?degree of listening loss and the relationship between curative effect and prognosis.The curative effect isjugdedby average of air conduction hearing threshold which is tested by pure tone audiometry.We also research the frequency distribution and degree of hearing loss of pure tone audiometry of patients of SSNHLwith diabetes orhyperlipemia.Evaluation criteria: patients of SSNHLwith diabetes orhyperl ipemia are divided intothe observe group,the otherswhich excludethe other diseases of circulatory system are divided intothe control group.We compareand analyze the differences and its meaning of treatment program andhealing effect between the observe groups and the control group,and analysisage?sex?onset time?degree of listening loss and the relationship between curative effect and prognosis.Results:Theaverage age of diabetesgroup is 55.1?hyperlipidemia group is 48.3?the control group is 44.7;Averagelength of stay: diabetes group is 14.5 days,hyperlipidemia group is 15.7 days,control group is 12 days;when all the patients are grouped byage(bounded by the age of 40),P=0.08,the age is younger,the curative effect is better;whengrouped bymale or female,P=0.892,the curative effect is not related with sex;whengrouped bythe period from the onset to hospitalization(bounded by 3 days),P=0.000,the period of time from the onset to hospitalization is shorter,the curative effect is better;when grouped by with or without diabetes,P=0.002,the curative effect is worse in the patients with diabetes;when grouped by with or without hyperlipidemia,P=0.007,the curative effect is worse in the patients with hyperlipidemia.In thediabetes group,high frequency hearing loss is in the majority(53.6%);Thediabetes group are mainlysevere hearing loss or worse(78.6%);The hyperlipidemia group are mainly moderate or severe and hearing loss(70%).Conclusion:The average ages of diabetes and hyperlipidemiagroup are higher than the control group;The curative effect has no relation with sex.There is statistically significant between age ? the period from the onset to hospitalization ? with or without diabetes or hyperlipidemia and curative effect;Gender and no significant curative effect;In thediabetes group,high frequency hearing loss is in the majority;Thediabetes group are mainlysevere hearing loss or worse;The hyperlipidemia group are mainly moderate or severe and hearing loss;The course is long and prognosis is poor of th e patients with diabetes or hyperlipemia.To the patients with diabetes or hyperlipemia we should take integrate treatment programmes by control the primary disease. |
