The Evaluation Index System Establishment On Nursing Quality Of Third Grade Class Comprehensive Military Hospital

ObjectiveThis study aims to establish a comprehensive evaluation index system of Third Grade Class comprehensive military hospital nursing quality by Donabedian's model.This comprehensive evaluation index system included the evaluation indexes on nursing quality of field medical team and Level ? peacekeeping hospital.This study used research method of qualitative method and Delphi method to realize systematic and standardized clinical nursing quality management in Third Grade Class comprehensive military hospital.MethodThe first step,semi-structured interviews were conducted among 19 clincial nurses and 12 hospitalized patients with adoption of purposive sampling method.Using phenomenological method analyzed the data,and chosen the findings which related to the evaluation index to modify the evaluation index system.The second step,select personnel from Third Grade Class comprehensive military hospital,who participated in international peacekeeping medical support mission and make deep interview to the nursing quality situation of Level ? peacekeeping hospital nursing quality.Select contents related to medical support index in the results through phenomenological analysis and make the draft of military peacekeeping Level ? hospital medical support nursing quality index.The third step,consult with 30 nursing management experts from 19 Third Grade Class military hospital in Shandong and Henan by way of Delphi method and make two rounds of expert consultations.Excel2007 with SPSS21.0 for data analysis.Calculate the positive coefficient,authoritative coefficient and coordination coefficient of expert advice.The experts consulting reliability and representativeness were also tested.ResultsStage?: Qualitative research on the cognition of nursing quality evaluation by nurses and patients in military hospital.There are two themes to emerge.(1)Advantages and disadvantages of quality evaluation: Most of clinical nurses and patients believe that quality evaluation is good for patients and also improve nurses' self ability,which is good for them.While the unreasonable phenomenon existed in quality evaluation causes negative emotions between nurses and patients.(2)Connotation of nursing quality: Nursing quality evaluation should pay attention to the health condition and feelings of patients,nursing administrator needs to strengthen professional technology inspection at the same time need to pay attention to the humanized management.This study is concerned with the understanding and perception of the nursing quality standards executive and beneficiaries,highlight the human nature of the quality control process.This research provides guidance for the humanistic nursing quality evaluation index system in Third Grade Class comprehensive military hospital.Stage?: Qualitative research on nursing quality evaluation situation of Level ? peacekeeping hospital.Three themes and nine Sub themes were identified.The three themes were organization management,ward management and technical management.The sub-themes were organization structure,system criterion,education and exercise,environmental safety,material management,cultural communication,disinfection and isolation,occupational protection.It is used to provide reference for make the draft of Level ? peacekeeping hospital nursing quality index,and for the military hospital to establish different deployment modes(field medical team,field square cabin hospital,hospital ship)military action of medical support tasks nursing quality evaluation index to provide reference.Stage?: Establishment of nursing quality evaluation index system in Third grade class comprehensive military hospital.(1)This evaluation index system is divided into two parts.Part one is inpatient area comprehensive nursing quality evaluation,which includes 3 one-level indicators,27 two-level indicators,and 81 three-level indicators.The second part is medical support mission nursing quality evaluation index,in which the field medical team nursing quality evaluation index include 5 one-level indicators,28 two-level indicators and Level ? peacekeeping hospital nursing quality evaluation index includes 5 one-level indicators,33 two-level indicators.(2)The expert positive coefficient at first round is 96.67% and the second round is 96.55%,authority coefficient is 0.875.In addition,two rounds of expert opinion coordination degree value lower than 0.05 using chi-square test.The choosing of experts is reasonable with high credibility.ConclusionBy using qualitative study and Delphi method,this index system has good concentration in Third Grade Class comprehensive military hospital in improving nursing quality management and evaluation.The evaluation system provides useful reference to standardization of diverse military action in military hospital,nursing management in medical support mission and strengthening accompanying support capability and provides useful information for making army nursing development plan and improving military hospital nursing quality evaluation system.
