Study On PICC Nursing Quality Evaluation Index System In Hospital

PICC(peripherally inserted central catheter) is the central catheter insertedperipherally, which can be used for medium-term or long-term intravenous infusiontherapy longer than7days or the infusion of hypertonic drugs and irritation stronger drugs,and PICC can remain in situ for extended periods of time of a year. It is increasinglywelcomed by healthcare personnel and patients for the characteristics of ease of operation，low risk, few complications and long-term insertion. However, it’s also a nursing techniquewith difficult clinical operation and related complications. On the basis of consultinginternal or external medical literature about quality indexes, this project established ansystematic and scientific framework of PICC nursing quality evaluation index system withthe method of experts meeting and Delphi. Then, according to the nascent PICC nursingquality evaluation index system, the PICC application and management situation ofhospitals in which there are members of Intravenous infusion committee in ShanghaiNursing Association was analyzed using self-designed questionnaire of PICC NursingQuality Status of Shanghai Third-senior Hospital. This study focused on status of elementquality and terminal quality, and it was committed to find problems in PICC application inorder to provide decision support for health administration departments and hospitalmanagers. This paper was divided into two parts.Part One: On the basis of consulting internal or external medical literature aboutquality indexes and nursing care of PICC, this project established an systematic andscientific framework of PICC nursing quality evaluation index system with the method ofclinical investigation, experts meeting and Delphi. This index system included3first-classindexes,10second-class indexes and45third-class indexes. Statistical analysis showed,concentration degree of experts’ views and consensus of results of research were at a highlevel, and credibility of consulting results were also high (Ca=0.885).Part Two: according to the nascent PICC nursing quality evaluation index system, thePICC application and management situation of hospitals in which there are members ofIntravenous infusion committee in Shanghai Nursing Association was analyzed usingself-designed questionnaire of PICC Nursing Quality Status of Shanghai Third-seniorHospital. This study focused on status of element quality and terminal quality. Statisticsanalysis by statistical package of social sciences (SPSS)18.0showed, the PICC technologyof25hospitals in Shanghai began relatively late, but with a fast pace of development, andthere were5hospitals became the admitted PICC training base in Shanghai; PICC were professionally managed in various forms of professional group in these25hospitals; itcould be seen from tendency of test that population of insertion nurses was growing alongwith hospital’s beds scale;With the increase of the beds scale,more maintenance nurses ofnumber of hospitals are also increased(P=0.038);PICC insertion nurses qualificationaccessed should be improved further, all these25hospitals set access standards for PICCinsertion nurse; PICC related new technology were few, and materials and equipmentshould be improved; PICC related scientific research and academic exchanges should bestrengthened; the tracking and management for PICC insertion patients and developmentof databases needed to be improved. Only10of them have databases, however the datawas incomplete, which could not be used for further analysis of complications.
